Synergy Family Services, Inc . (SFS) is a dynamic Outpatient Mental Health Center focused on providing strengths based, culturally proficient services to enhance the social, emotional, behavioral health and wellness of individuals and families. Job Title: PRP Direct Care Staff

POSITION SUMMARY: The PRP Direct Care staff will promote recovery and resiliency by providing rehabilitation services designed to meet the individualized needs of each consumer while meeting with assigned consumers weekly, or as necessary, based on the individual needs of each consumer and agency policy. Bilingual (Male Preferred) candidates are a plus.
JOB RESPONSIBILITIES:
Attend all PRP Training Orientation.
Respond to emails and phone calls within 2 business days.
Complete required job training on site and/or off site.
Provide Human Resources documents when requested.
Attend all mandatory Synergy or PRP Program meetings.
Have a caseload of a minimum of 12 clients
Contacting client within 72 hours of assignment and write attempt in the memo section in the practice management system
Attempt to contact client at least 3 times in a two-week time period and document attempts in memo section
Inform PRP Supervisor and Program Director via email within 2 weeks of initial assignment if the client is not responsive.
Schedule youth clients’ for individual visits and have youth engage in psychoeducational groups weekly for a total of 4x per month.
Schedule adult clients’ for individual visits and have adults engage in psychoeducational groups weekly for a total of 6x per month.
Schedule and meet with adult clients twice per week for one hour.
FSW usage for DCS’s that are not bilingual for families that need translation services.
DCS must schedule ahead of time to include FSW in the introduction visit and all treatment plan reviews.
Perform rehabilitation activities in the SFS’ facility, in clients’ homes, and in the community
Work cohesively with administrative staff to market program/groups
Attend weekly supervision with Rehab Specialist/Supervisor the first month of employment, then bi-weekly supervision after that.
Complete PRP interest calls and report client information to PRP Supervisor and program director
Complete paperwork in accordance with COMAR standards.
Complete contact notes within 3 days of the session.
Individual Rehabilitation Plans (IRPs) within 14 days of intake that include client participation and signature. Individual Rehabilitation Plan reviews (IRPs) are completed every October and April (not to surpass 6 months) for adults and every October, January, April, and July (not to surpass 3 months) for children after the initial IRP. IRP's are expected to be completed to agency standard and are subject to re-do to be considered completed and in compliance. Changes should be made within 48 hours of request.
Transition Requests, initially, are completed within the 3 rd session, when a client is transferred (within 10 days), and when a client is discharged (within 10 days)
Monthly documented Collaboration with each client’s therapist in the Monthly Collaboration Note by the 15th of every month.
Inform PRP Supervisor via email of requested discharges no later than 24 hours and complete discharge paperwork (PRP Discharge Summary Note and Transition Summary) within 10 calendar days of request
Report grievances or incidences immediately
Any paperwork corrections must be submitted within three days of the request
Monthly Billing Summaries must be submitted by 11:59pm on the 3 rd regarding billables provided the month prior.
Invoices and monthly billing summaries are to be emailed to Supervisor and PRP Director by the 3 rd of the month after services were provided. They must be approved by Supervisor first before being processed and sent to accounting/bill.com
All staff will participate in a monthly meeting facilitated by the PRP Director the 3rd Wednesday of every month from 10am-12pm.
Other administrative PRP tasks, if needed
QUALIFICATION AND EDUCATION: Direct Care Staff will possess a Bachelor degree in a related field; or Associate degree in a related field; or At least 30 credit hours of college credit toward a bachelor degree in a related field; or At least 1 year of work experience in a supervised mental health setting. Qualified applicants looking to work with minors will also have at least 2 years of paid work experience serving children and adolescents. Bilingual (Male Preferred) candidates are a plus.
