    "requisitionDescription": "<div><p style=\"text-align: center;\">Primary Care Access Network Navigator</p><p id=\"isPasted\"><u>JOB SUMMARY</u></p><p>Under the direction of the PCAN Program Manager and Florida Hospital East Director of Case management, the PCAN Navigator will outreach, educate, and facilitate the enrollment of uninsured patients into PCAN &quot;medical homes.&quot; In addition, the PCAN Navigator will perform administrative and casework duties in interviewing applicants to determine eligibility for program assistance and link community residents with appropriate service agencies as needed.</p><p><u>PRIMARY FUNCTIONS</u></p><ol start=\"1\" type=\"1\"><li>Provide outreach/education to patients, families, caregivers, social workers/case managers, etc. in designated zip code areas to include Florida Hospital East and Health Fairs.</li><li>Interview potential clients and determine their eligibility for participation according to program guidelines.&nbsp;</li><li>Exercise independent judgement in making eligibility determination, determining financial status, screening for other resources, and overall suitability of applicant program. Will also complete necessary paperwork/online application for eligible clients into Medicaid and Medicare programs.</li><li>Ensure regular/timely follow-up with clients to determine outcomes of enrollment process.</li><li>Solve routine problems, answer questions, and respond to inquiries in a timely manner.</li><li>Attend and actively participate in regularly scheduled workgroups, including but not limited to, PCAN Case Management, school nurse meetings, etc.</li><li>Compile files, maintain logs, and assist with scheduling appointments as necessary.</li><li>Prepare monthly reports outlining program statistics (number of clients served, number of health screenings provided, etc.).</li><li>Other responsibilities as assigned</li></ol><p><u>EDUCATION AND EXPERIENCE</u> &nbsp;</p><ol start=\"1\" type=\"1\"><li>Associates Degree from an accredited institution of higher learning</li><li>Two (2) years of experience in social, health, or public services, or an equivalent combination of relevant education, training, and experience</li></ol><p><u>KNOWLEDGE, SKILLS, AND ABILITIES</u> &nbsp;</p><ol start=\"1\" type=\"1\"><li>Computer competency</li><li>Knowledge of word processing and spreadsheet applications</li><li>Ability to work well with people</li></ol><p><u>ADDITIONAL QUALIFICATIONS</u>&nbsp;</p><ol start=\"1\" type=\"1\"><li>Bilingual a plus</li></ol><p><u>RELATIONSHIP REPORTING</u>&nbsp;</p><ol start=\"1\" type=\"1\"><li>Reports to the PCAN Program Manager and Florida Hospital East Director of Case Management</li></ol><p><u>PHYSICAL REQUIREMENTS</u>&nbsp;</p><ol start=\"1\" type=\"1\"><li>Ability to stand, walk, or view a computer screen for extended periods of time</li><li>Ability to use hands and fingers to handle or feel</li><li>Ability to reach with hands or arms</li><li>Ability to perform repetitive hand and wrist motions for extended periods of time</li><li>Ability to effectively talk and listen to patients</li><li>Ability to work independently and off-site</li></ol></div>\n",
