Adjusts treatment plan to achieve maximum results in independence and/or function</li>\n <li>Provides empirically based psychological interventions to the patients and families as appropriate for their specific needs to maximize and encourage their participation with the community at large</li>\n <li>Designs community reintegration activities to assist the patient with their psychological/cognitive needs during physical activity</li>\n <li>Assists the patient in understanding their needs and limitations and how to manage them during function</li>\n <li>Regularly re-assesses effectiveness of treatment plan, progress of patient towards goals and the need for modifying goals or treatment</li>\n <li>Communicates patient’s needs and progress to the treatment team, physician, patient, family members, and others as applicable</li>\n <li>Consults with medical providers regarding patient evaluation & treatment, progress of the patient & discharge from treatment</li>\n <li>Observes/documents and reports changes in the condition/status of the person receiving services to the Clinical Coordinator, Program Manager or DCM, attending physician and other treatment team members</li>\n <li>Maintains and submits evaluation, treatment and billing documentation in the format and timelines required by the Company, facility, funding source, local, state and federal regulations</li>\n <li>Adheres to standards of the Company's Code of Business Conduct, policies, HIPAA and Corporate Compliance programs and appropriate professional practice standards and applicable state/federal laws</li>\n <li>Performs other duties as assigned</li>\n</ul>\n<h2>Qualifications</h2>\n<ul>\n <li>Master’s or doctoral degree in counseling or a counseling-related field from an accredited college or university, as determined by the state licensing requirements</li>\n <li>Current professional license and/or certification as required by the state of practice</li>\n <li>A minimum of one year of post master’s degree clinical experience and counseling individuals with traumatic or acquired brain injuries or related neurological conditions</li>\n <li>Knowledge of complex neurologic injury rehabilitation techniques</li>\n <li>Demonstrates effective and professional interpersonal, verbal, and written communication skills</li>\n <li>Able to work independently and part of a team</li>\n <li>Has reliable transportation and can travel to/from multiple assignments/ locations on a daily basis</li>\n</ul>\n<h2>About our Line of Business</h2>Rehab Without Walls, an affiliate of BrightSpring Health Services, is a revolutionary neurorehabilitation program that moves individuals outside the walls of institutional settings and into their natural home and community environment through a continuum of care.
