    "ExternalDescriptionStr": "<p>Park Nicollet is looking to hire a Registered Nurse to join our Home Care team!&nbsp; Come join us as a Partner for Good and help us make an impact on the care and experience that our patients and their families receive every day.&nbsp; &nbsp;</p>\n<p><strong><u>Position Summary:</u></strong></p>\n<p>The Home Care registered nurse assesses and evaluates patient care needs, provides skilled nursing care and teaching to patients and families in their home or community - based upon community standards of practice.&nbsp;</p>\n<p>The nurse is responsible to improve or maintain patient’s health status, provide terminal illness care, and to prepare patients and their families for self-directed and/or assisted care in their community setting.&nbsp;</p>\n<p>The nurse coordinates medical, social, and financial resources to develop a comprehensive plan of care.&nbsp; The nurse supports the following community care processes:&nbsp; admission, discharge, billing, referrals, bereavement, and care delivery.</p>\n<p><strong><u>Work Schedule:</u></strong> M-F 8a-4:30p, occasional evenings and on-call. Every 6-7<sup>th</sup> weekend. 1-2 holidays/year.</p>\n<p><strong><u>Territory:&nbsp;</u></strong></p>\n<p>Central: (ex: St. Louis Park, Minneapolis, Edina, Brooklyn Center)</p>\n<p><strong><u>Required Qualifications:</u></strong></p>\n<ul>\n <li><strong>Education, Experience or Equivalent Combination:</strong>\n  <ul>\n   <li>Work experience as a RN for at least two years<strong>&nbsp;</strong></li>\n  </ul></li>\n <li><strong>Licensure/ Registration/ Certification:</strong>\n  <ul>\n   <li>Current MN Registered Nursing License&nbsp;</li>\n  </ul></li>\n <li><strong>Knowledge, Skills, and Abilities:</strong>\n  <ul>\n   <li>Demonstrates knowledge of age-related growth and development principles necessary to provide appropriate service and assure safety of patients.</li>\n   <li>Proven proficiency in the operation of the following: Personal Computer, Telephone, Fax Machine, Photocopier, Printer, and electronic medical record.</li>\n  </ul></li>\n</ul>\n<p><strong><u>Preferred Qualifications:</u></strong></p>\n<ul>\n <li>Experience in home care and or hospice desirable.</li>\n <li>Must be able to function independently with minimal supervision.&nbsp;</li>\n <li>High level of sensitivity, assessment skills, technical-skills and communications skills.&nbsp;</li>\n</ul>\n<p>Demonstrates initiative, judgment, critical thinking, and the ability to problem solve.&nbsp;</p>\n<p>&nbsp;</p>\n<p><strong><u>Benefits:</u></strong></p>\n<p>Park Nicollet offers a competitive benefits package (for eligible positions) that includes medical insurance, dental insurance, a retirement program, time away from work, insurance options, tuition reimbursement, an employee assistance program, onsite clinic and much more!&nbsp;</p>",
