    "description_html": "<p>Professional Medical Practice, P.C., is looking for an <strong>Alcohol and Substance Abuse Counselor</strong> (CASAC-T, CASAC, CASAC2) to work in the medical department at the <strong>Sullivan County Jail</strong> located in <strong>Monticello, NY</strong></p><p><br></p><p>This is a full-time position, working 40 hours per week, Monday-Friday (8:00am-4:30pm).</p><p>Hourly Rate: $25.00- $28.00.</p><p>You must have a <strong>CASAC-T, CASAC, or CASAC2 </strong>to qualify for this role.</p><p><br></p><p><u><strong>Job Responsibilities:</strong></u></p><ul><li>Responsible for the clinical monitoring, coordination, and implementation of the behavioral health program in accordance with Company policies and procedures.</li><li>Serve as the liaison between medical and facility staff as it pertains to Medication Assisted Treatment (MAT).</li><li>Engage patient in therapeutic interventions with the goal of addressing substance use related concerns.</li><li>Facilitate group and individual sessions with patients in the MAT program.</li><li>Actively assist with the detection and prevention of patient suicide.</li><li>Document all patient encounters in a timely manner utilizing the SOAPE note format.</li><li>Assist and/or collaborate with facility staff regarding discharge planning for patients in the MAT program, where applicable.</li><li>Facilitate/participate in interdisciplinary team meetings and/or case consultations with staff across disciplines.</li><li>Participate as a member of the Quality Assurance Committee and any sub-committees. Assist in the establishment of Quality Assurance standards and audit criteria.</li><li>Serve as a member of various committees within the medical unit and facility as necessary.</li></ul><p><u><strong>Benefits</strong></u><strong>:</strong></p><p>Professional Care believes in offering a competitive compensation and benefits package to all their employees! You are also eligible to accrue “sick and safe” leave benefits under the New York State Sick Leave Law (i.e., NYSSL).
