This role emphasizes <strong>holistic, person-centered care</strong> guided by SAMHSA’s 8 Dimensions of Wellness—emotional, social, physical, financial, and more.</p><p>You’ll work as part of a <strong>multi-disciplinary team</strong>, using psychiatric rehabilitation and integrated care models to support both mental and physical health outcomes.</p><p><br></p><h3>What You’ll Do</h3><ul><li>Build strong, therapeutic relationships with clients—including those who may be harder to engage&nbsp;</li><li>Help clients set and achieve personalized wellness goals&nbsp;</li><li>Provide skill-building and psychoeducational support (individual &amp; group settings)&nbsp;</li><li>Coordinate care plans and connect clients to essential community resources&nbsp;</li><li>Support clients and families in making informed decisions&nbsp;</li><li>Assist with access to medical, behavioral health, and social services&nbsp;</li><li>Provide crisis intervention when needed&nbsp;</li><li>Maintain accurate documentation and meet service delivery standards&nbsp;</li></ul><h3>What We’re Looking For</h3><ul><li>Strong interpersonal and communication skills&nbsp;</li><li>Passion for mental health, community support, and advocacy&nbsp;</li><li>Ability to work collaboratively in a team environment&nbsp;</li><li>Flexibility and willingness to meet clients where they are (including community-based work)&nbsp;</li><li>Experience with behavioral health, case management, or human services preferred&nbsp;</li></ul><h3>Work Environment</h3><p>This is a dynamic, community-based role requiring travel to client homes, service sites, and community locations.
