This role requires strong independent judgment, relational awareness, and the ability to translate framework into action across varied community contexts.</p><p><br></p><p style=\"margin-left:0in;\">Serving as a program officer, subject matter expert, and coalition coach, the Learning Accompaniment Lead facilitates conversations that may include tension or competing priorities while maintaining alignment with PJTT principles. Autonomy is expected in tailoring approaches to community strengths, needs, and emerging challenges.</p><p><br></p><p><strong>Interactions / Working Relationships</strong></p><div style=\"margin-left:0in;\"><ul style=\"list-style-type: disc;margin-left: 0in;\"><li style=\"margin-left:0in;\">Frequent collaboration with PJTT and Communities of Solutions (CoS) team members.</li><li style=\"margin-left:0in;\">Regular engagement with community coalitions, Community Connectors, persons with lived experience, faith communities, schools, nonprofits, and other external stakeholders.</li><li style=\"margin-left:0in;\">Partnership with internal MHM departments to integrate health equity practices and advance community-driven initiatives</li></ul></div><p><strong>&nbsp;</strong></p><p><strong>Essential Duties and Responsibilities</strong></p><div style=\"margin-left:0in;\"><ul style=\"list-style-type: disc;margin-left: 0in;\"><li style=\"margin-left:0in;\">Manage philanthropic interactions by developing trust-based relationships with coalitions, providing guidance, monitoring progress, and engaging MHM departments.</li><li style=\"margin-left:0in;\">Deliver presentations on PJTT frameworks (e.g., Pathways to Population Health, driver diagrams, 90-day action planning) across diverse settings.</li><li style=\"margin-left:0in;\">Serve as a subject matter expert advising coalitions on PJTT frameworks.</li><li style=\"margin-left:0in;\">Provide monthly guidance to coalitions to support strategy development and implementation.</li><li style=\"margin-left:0in;\">Collaborate with PJTT team members, Community Connectors and other stakeholders in co-designing and co-evaluating PJTT initiatives.</li><li style=\"margin-left:0in;\">Develop and deploy capacity-building resources such as trainings and toolkits.</li><li style=\"margin-left:0in;\">Facilitate community conversations, ensuring inclusion of diverse voices, especially people with lived experience.</li><li style=\"margin-left:0in;\">Build and maintain strong external partnerships with community organizations, faith groups, and schools.</li><li style=\"margin-left:0in;\">Collect, analyze, and report coalition progress using both quantitative and qualitative data.</li><li style=\"margin-left:0in;\">Contribute to PJTT curriculum development and participate in tactical planning.</li><li style=\"margin-left:0in;\">Prepare coalitions to enter the PJTT Implementation Phase through continually assessing their readiness.</li><li style=\"margin-left:0in;\">Participate in the planning, implementation, and continuous improvement of PJTT outreach in designated regions and the Learning Phase application process.</li><li style=\"margin-left:0in;\">Participate in the planning, implementation, and continuous improvement of PJTT events.</li><li style=\"margin-left:0in;\">Perform other duties as assigned.</li></ul></div><p>(Approximate % of role: Coalition support and coaching 40%;
