Join us in making a real difference in the health of our community.</em></p>\n<h2>Job Summary</h2>\n<p><strong>Job Summary: </strong>We are seeking a Cardiovascular Nurse Practitioner with at least one year of outpatient cardiology experience to support a growing cardiology service line. This role works collaboratively with cardiologists and clinical staff to provide high-quality, patient-centered cardiovascular care in an outpatient setting.</p>\n<h2>Responsibilities</h2>\n<p><strong>Responsibilities include:</strong></p>\n<ul>\n <li><p>Conduct comprehensive cardiovascular assessments, including history, physical exams, and risk stratification</p></li>\n <li><p>Manage and follow patients with cardiac conditions such as hypertension, heart failure, coronary artery disease, arrhythmias, and hyperlipidemia</p></li>\n <li><p>Order, interpret, and review diagnostic tests (EKGs, echocardiograms, stress tests, Holter monitors, labs)</p></li>\n <li><p>Develop and implement treatment plans in collaboration with cardiology physicians</p></li>\n <li><p>Prescribe medications and adjust therapies in accordance with evidence-based guidelines</p></li>\n <li><p>Provide patient education on disease management, lifestyle modification, and medication adherence</p></li>\n <li><p>Document clinical encounters accurately in the electronic medical record</p></li>\n <li><p>Coordinate care with referring providers, imaging, and ancillary services</p></li>\n <li><p>Participate in quality improvement initiatives and clinical protocols</p></li>\n</ul>\n<h2>Qualifications</h2>\n<p><strong>Required</strong></p>\n<ul>\n <li><p>Active Nurse Practitioner license in NY</p></li>\n <li><p><strong>Minimum of 1 year of outpatient cardiology Nurse Practitioner experience</strong></p></li>\n <li><p>Board certification (ANCC or AANP)</p></li>\n <li><p>Active DEA (or eligibility to obtain)</p></li>\n <li><p>Strong knowledge of cardiovascular disease management</p></li>\n <li><p>Excellent communication and clinical judgment skills</p></li>\n</ul>\n<p><strong>Preferred</strong></p>\n<ul>\n <li><p>Experience in outpatient cardiology or multispecialty practices</p></li>\n <li><p>Familiarity with EPIC or similar EMR systems</p></li>\n <li><p>Experience managing high-volume patient panels</p></li>\n</ul>\n<h2>Equal Opportunity Employer</h2>\n<p><strong><em>Essen Health care is proud to be an equal opportunity employer, and we seek candidates who desire to work in and serve an ethnically diverse population.</em></strong></p>",
