Description

RN Field Nurse Position Description: The RN Field Nurse, under the direction of the Supervising Nurse, is responsible for comprehensive care to patients through the activities of assessment, planning, and provision of direct nursing care. The Staff Nurse makes the initial patient evaluation and preforms start of care (SOC) visits. Essential Duties and Responsibilities: 1. Assumes responsibility for an assigned caseload of patients to a developed plan of care. 2. Conducting a comprehensive, fact-finding nursing assessment and plan of care via a full medical & psychosocial assessment including a nursing assessment of body systems and co-morbidities. 3. Developing/follow an established nursing rehabilitation care plan with ongoing assessment, actions, implementations and measurement of outcomes. Provide revisions to maintain medical stability. 4. Guide the patient and family/guardian by providing oversite of all medical care to promote independence, wellness and illness prevention through education and self-empowerment. 5. Collaborate with the patient, family/guardian, and the rehabilitation/care team (therapists, aides, doctors etc.) to ensure appropriate continuity of care and facilitate home modifications. 6. Monitoring and evaluating all services provided to insure that infection control, safety precautions and medical requirements are met and the patient is continuously moving towards expected goals. 7. Prepares and maintains clinical documentation according to Agency policies and acceptable nursing standards including ongoing daily case activity from SOC to discharge in a chronological timeline. 8. Develops, implements and updates comprehensive, goal focused care plan with preventative and rehabilitation nursing procedures for patients. 9. Makes planned home visits to the patient for assessment, instruction, supervision, and provision of nursing treatments according to the plan of care. Provides instruction/guidelines to aides providing services, including ongoing supervision and evaluation of the services delivered. 10. Shares the responsibility of on call duties in emergency and/or staffing situations that may involve providing “off hours” nursing visits or shift work. 11. Initiates and maintains continuous contact with the patient’s physician to report the patient’s response to home care, any changes in status, or possible need to additional services. 12. Interprets agency services and policies to patients, families, physicians, community services as well as legal responsibilities of patient care, rights and property. 13. Participates in orientation, in-service programs, staff meetings, case re-assessments and conferences as well as maintains CEU and RN competencies. 14. Maintains productivity according to work status, organizes daily visit schedule to minimize travel. 15. Informs the Supervising Nurse of patient services provided and needed and any changes needed or made to the plan of care or to the patient schedule. 16. Performs other duties as assigned Qualifications: 1. Minimum of two years of nursing in an acute care or home care setting is preferred. 2. Meet the physical demands of providing nursing care, ambulating patients, transferring patients (proper techniques& lifting u p to 50 lbs ), assisting with exercises and promote safety and well-being. 3. Current licensure as a registered nurse in Michigan, a Michigan driver’s license, proof of automobile insurance and reliable transportation and able to pass a background check and drug screen. 4. Must have excellent communication skills as well as the maturity for establishing and maintaining positive relationships with patients, families, and agency personnel. 5. Must be empathetic, non-judgmental, tactful, responsible, adaptable, organized, and committed to the purpose and objectives of the organization. #EXTMONTHLY
