The requirements listed below are representative of knowledge, skill and/or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.</p><p><br></p><p>· Conduct in-home or facility-based visits to hospice patients for initial assessments, face-to face visits, routine follow-up visits, and symptom management.</p><p>· Collaborate with the Medical Director or Associate Medical Director and the IDT to develop, implement, and revise individualized care plans.</p><p>· Provide expert pain and symptom management consistent with hospice philosophy and guidelines.</p><p>· Serve as a clinical resource to hospice nurses and other staff.</p><p>· Participate in IDT as needed.</p><p>· Educate patients and families on disease progression, end-of-life expectations and hospice treatment options.</p><p>· Document all patient care accurately and timely in the electronic medical record.</p><p>· Participate in on-call coverage as needed.</p><p>· Comply with company policies, procedures, and guidelines.</p><p><br></p><p><br></p><p><strong>Location:</strong> La Grange, Texas</p><p><strong>Shift:</strong> PRN- up 12 hours per week</p><p><strong>Benefits:</strong> 403b Retirement and mileage reimbursement!</p>",
