As a Primary Care Registered Nurse (RN), you will play a pivotal role in providing comprehensive healthcare services to patients within a primary care setting. Your expertise will be essential in promoting health education, assessing patient needs, and coordinating healthcare services to ensure optimal patient outcomes.</p>\n<p> </p>\n<p><strong>Shifts:</strong> Nurses can expect to work 8 hour shifts M-F.</p>\n<h2>Primary Responsibilities</h2>\n<ul>\n <li><p><strong>Patient Assessment and Care:</strong> Conduct thorough initial patient assessments, including health history, physical examinations, and symptom evaluation. Monitor and manage patient conditions, addressing acute and chronic illnesses, and collaborating with physicians on treatment plans.</p></li>\n <li><p><strong>Health Education and Counseling:</strong> Educate patients and families on preventive care, wellness strategies, and treatment options. Provide guidance on medication usage, nutrition, and lifestyle modifications to promote overall health and well-being.</p></li>\n <li><p><strong>Care Coordination:</strong> Coordinate patient care activities, including referrals to specialists, diagnostic tests, and follow-up appointments. Ensure seamless transitions between healthcare settings and maintain communication with interdisciplinary team members.</p></li>\n <li><p><strong>Documentation and Record Keeping:</strong> Maintain accurate and detailed patient records, including medical histories, symptoms, treatments, and outcomes. Adhere to regulatory requirements and organizational policies for documentation and reporting.</p></li>\n <li><p><strong>Patient Advocacy:</strong> Advocate for patients' rights and preferences, ensuring their voices are heard in healthcare decision-making processes. Serve as a liaison between patients, families, and healthcare providers to facilitate open communication and shared decision-making.</p></li>\n <li><p><strong>Quality Improvement:</strong> Participate in quality improvement initiatives to enhance patient care outcomes and operational efficiencies. Contribute to evidence-based practice guidelines and clinical protocols.</p></li>\n <li><p><strong>Emergency Response:</strong> Assess and respond to medical emergencies within the scope of practice, providing immediate care and initiating emergency protocols as needed.</p></li>\n</ul>\n<h2>Minimum Qualifications</h2>\n<ul>\n <li><strong>Licensure:</strong> Current, unrestricted RN license from any state in the United States.</li>\n <li><strong>Certification:</strong>  Current Cardiopulmonary Resuscitation (CPR)/Basic Life Support (BLS) certificate accredited by the American Heart Association.  </li>\n <li><strong>Experience:</strong> Minimum of three (3) years prior experience in a primary care setting</li>\n <li><strong>Education:</strong> Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by ACEN or CCNE.</li>\n <li>Ability to perform under pressure in emergency situations.</li>\n <li>Excellent documentation and communication abilities.</li>\n <li>Team-oriented with a focus on collaborative patient care.</li>\n <li>Ability to complete and successfully pass any required government background check</li>\n</ul>\n<h2>About Aptive</h2>\n<p>Aptive is a modern federal consulting firm focused on human experience, digital services, and business transformation.
