Across our network of community centers, we empower people of all backgrounds to flourish, recognizing the intrinsic value within each individual and the importance of building community.</span></p><p><span style=\"color: rgb(0, 0, 0);\"><strong>JOB PURPOSE</strong></span></p><p><span style=\"color: rgb(0, 0, 0);\">Counsel and aid individuals who have substance use disorders in Pride Site, a New York State OASAS-certified residential treatment program&mdash;one of four programs in the newly constructed Center for Recovery and Wellness (CRW).</span></p><p><span style=\"color: rgb(0, 0, 0);\"><strong>SCOPE OF INFLUENCE</strong></span></p><ul type=\"disc\"><li style=\"color: rgb(0, 0, 0);\">Provide individual case management and group counseling to adults with substance use diagnoses (and their families/significant others).</li><li style=\"color: rgb(0, 0, 0);\">Partner with referral sources, providing regular updates and progress on clients.</li><li style=\"color: rgb(0, 0, 0);\">Partner with a multi-disciplinary team in assessing diagnoses, planning treatment, providing coverage of clients&rsquo; individual and group sessions, and offering clinical insight and expertise.</li></ul><p><span style=\"color: rgb(0, 0, 0);\"><strong>KEY RESPONSIBILITIES AND ACCOUNTABILITIES</strong></span></p><p><span style=\"color: rgb(0, 0, 0);\"><strong>Clinical and Documentation (80%)</strong></span></p><ul type=\"disc\"><li style=\"color: rgb(0, 0, 0);\">Provide up to 20 clients with biweekly individual case management sessions to continuously assess appropriateness of current level of treatment.</li><li style=\"color: rgb(0, 0, 0);\">Coordinate services with medical, mental health, and other social service providers; communicate with the on-site outpatient program to ensure clients on caseload are receiving and responding to clinical care in the outpatient program while residing in the treatment facility.</li><li style=\"color: rgb(0, 0, 0);\">Arrange and participate in multidisciplinary interventions for clients on caseload who may be struggling at the current level of care and follow up by assessing progress incrementally.</li><li style=\"color: rgb(0, 0, 0);\">Provide evidence-based group counseling to clients in the residential program.</li><li style=\"color: rgb(0, 0, 0);\">Complete detailed progress notes, comprehensive treatment plans, psychosocial assessments, discharge plans, and other required documentation in accordance with standards.</li><li style=\"color: rgb(0, 0, 0);\">Maintain accurately and organized clinical charts, engaging in pre-audits of charts to ensure compliance with OASAS and managed care standards.</li><li style=\"color: rgb(0, 0, 0);\">Send weekly, monthly, and quarterly reports to HRA through the Substance Abuse Tracking and Reporting System.</li><li style=\"color: rgb(0, 0, 0);\">Check the toxicology reporting portal daily to ensure any positive toxicology reports are communicated to supervisory staff, documented, and addressed with the resident.</li><li style=\"color: rgb(0, 0, 0);\">Ensure that clients&rsquo;
