Job Summary: In this role you’ll be responsible for telephone and walk-in triage as well as assisting medical providers and clinical staff with direct patient care such as wound care, ostomy cares, patient education, and other duties within the scope of your practice. Schedule: Anticipating 830-5pm, Mondays/Thursday/Fridays working at the clinic. Qualifications: Valid Wisconsin RN licenses and BLS certification, required. Minimum of 3 to 5 years clinical experience, preferred. Triage experience, preferred. Responsibilities include: Conducting initial assessments over the phone or in person to evaluate the patient's medical concerns. Using clinical knowledge and triage protocols to determine the urgency of the patient’s condition. Asking detailed questions about symptoms, medical history, and any other relevant information to assess the need for immediate care. Prioritizing cases based on the severity of symptoms and medical history. Providing immediate care instructions for non-emergency situations, such as home care advice or medication recommendations. Referring patients to a physician, nurse practitioner, or other healthcare provider as needed for further evaluation and care. Educating patients on self-care measures and treatment options when appropriate. Offering instructions for at-home care, medication management, and lifestyle modifications as per medical protocols. Providing information on preventive care, health screenings, and follow-up appointments. Scheduling appointments or refer patients to specialists, urgent care, or the emergency room if necessary. Following up with patients to check on their condition and ensure proper care and treatment are being followed. Maintaining accurate and detailed records of patient interactions, assessments, and actions taken. Communicating patient concerns and assessments with the healthcare provider or team. Working closely with physicians, nurse practitioners, and other medical staff to ensure effective care and patient safety. Documenting all patient interactions and decisions according to healthcare protocols and regulations. Ensuring all triage activities are in compliance with health regulations, standards of care, and organizational policies. Maintaining confidentiality and adhere to HIPAA (Health Insurance Portability and Accountability Act) guidelines. Knowledge, Skills, and Abilities: Demonstrates OMC's core behavioral standards and values in daily work. Ability to assess and make quick, accurate decisions about patient care based on symptoms and medical history. Strong verbal and written communication skills to effectively interact with patients, families, and the healthcare team. Familiarity with medical triage systems and guidelines. Ability to prioritize multiple tasks and manage a high volume of patient interactions. Ability to provide reassurance and comfort to patients while addressing their concerns. Physical Requirements for the Role: Ability to sit for extended periods while performing telephone triage and computer documentation. Frequent use of hands and fingers for keyboarding, telephone operation, and other office equipment. Ability to communicate effectively verbally and in writing with patients, providers, staff, and external agencies. Ability to hear and comprehend conversations over the telephone and in person. Visual acuity sufficient to read electronic medical records, computer screens, and written materials. Ability to occasionally stand, walk, bend, reach, and stoop within the clinic setting. Ability to occasionally lift, carry, push, or pull up to 20 pounds. Ability to maintain concentration and multitask in a fast-paced environment with frequent interruptions. Ability to respond appropriately and efficiently in urgent or emergent situations. Manual dexterity is sufficient to operate standard office and clinical equipment. Ability to work prolonged periods at a computer workstation. Work Environment: Primarily indoor clinic or office environment. Frequent telephone and computer-based patient interaction. Exposure to communicable diseases and other conditions common in healthcare environments. Possible exposure to stressful situations involving urgent patient concerns.
