You’re looking in the right place.</p>\n<p> </p>\n<p>As a <strong>Community Care Coordinator</strong>, you will assess and determine patient care needs and eligibility, provide access and referrals to community services, and engage with patients, caregivers and other health care practitioners.  </p>\n<p> </p>\n<p>You will play a lead role in providing connected, accessible, patient-centred care – and be supported by our collaborative team that includes over 9,000 regulated health care and other professionals. </p>\n<p> </p>\n<p>As a valued team member, your mission will be to help our patients be healthier at home, while you benefit from our supports for professional growth, personal wellness and work-life balance.</p>\n<p> </p>\n<p>Please note while you may be assigned to one primary location you may be required to be flexible and assist at other locations as needed.</p>\n<p> </p>\n<p><strong>What will you do?</strong></p>\n<ul>\n <li>In collaboration with patients and their families, assess care needs, determine eligibility for services, and develop individual care and service plans</li>\n <li>Link patients with service providers</li>\n <li>Coordinate and monitor care plan delivery</li>\n <li>Establish a helping relationship with patients and their families</li>\n <li>Balance patient needs and choices with available resources, ensuring patients’ values and preferences are respected</li>\n</ul>\n<p><strong> What must you have?</strong></p>\n<ul>\n <li>Membership, in good standing, with the applicable regulatory body:</li>\n <ul>\n  <li>College of Nurses of Ontario</li>\n  <li>College of Physiotherapists of Ontario</li>\n  <li>College of Occupational Therapists of Ontario</li>\n  <li>Ontario College of Social Workers and Social Service Workers</li>\n  <li>College of Dietitians of Ontario</li>\n  <li>College of Audiologists and Speech Language Pathologists of Ontario</li>\n </ul>\n <li>1+ years of recent experience in community health or a related field</li>\n <li>Knowledge of the health care delivery system and community resources</li>\n <li>Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills</li>\n <li>Effective time management, prioritization and organizational skills, with the ability to work independently and co-operatively in a busy multidisciplinary environment</li>\n <li>Established ability to accurately complete required documentation, reports and forms</li>\n <li>A valid driver’s license and access to a reliable vehicle</li>\n <li>Proficient in a Windows environment</li>\n <li>We have a mandatory COVID-19 vaccination policy.
