You will support with administrative tasks, including scheduling appointments and communicating with third-party funders.</span></span></p>\n<p><span><span style=\"font-size: 15px\"> </span></span></p>\n<p><span><span style=\"color: #222222; font-size: 15px\">You’ll work in a fast-paced environment as an integral member of a dynamic multidisciplinary team, while sharing our core values of communication, community, excellence, health, and innovation.</span></span></p>\n<p><span><span style=\"color: #222222; font-size: 15px\"> </span></span></p>\n<p><span><span style=\"font-weight: bold\"><span style=\"color: #222222; font-size: 15px\">Accountabilities</span></span></span></p>\n<ul>\n<li><span><span style=\"color: #222222; font-size: 15px\">Schedule and complete orientation sessions for clients when initiating treatment and ensure smooth transition of clients into program.</span></span></li>\n<li><span><span style=\"color: #222222; font-size: 15px\">Provide thorough assessment and exercise prescription for clients within a holistic approach.</span></span></li>\n<li><span><span style=\"color: #222222; font-size: 15px\">Support intake coordinator in managing WCB waitlist and provide information regarding program.</span></span></li>\n<li><span><span style=\"color: #222222; font-size: 15px\">Design rehabilitation plans for clients with physical injuries with the goal of returning client to pre-accident level.</span></span></li>\n<li><span><span style=\"color: #222222; font-size: 15px\">Complete functional testing evaluation and re-assessments as indicated.</span></span></li>\n<li><span><span style=\"color: #222222; font-size: 15px\">Assist in providing ongoing case management and client support.</span></span></li>\n<li><span><span style=\"color: #222222; font-size: 15px\">Facilitate one to one and group exercise and pain education sessions.</span></span></li>\n<li><span style=\"font-size: 15px\">Participate and facilitate weekly case consultations with the interdisciplinary team.</span></li>\n<li><span style=\"font-size: 15px\">Oversee client care, communicating progress, concerns, and outcomes with other clinicians.</span></li>\n<li><span style=\"font-size: 15px\">Communicate and collaborate with team members to ensure treatment plans are aligned.</span></li>\n<li><span style=\"font-size: 15px\">Ensure same day completion of session notes.</span></li>\n<li><span style=\"font-size: 15px\">Template and complete initial, progress, and discharge reports for funders.
