Unaccompanied Refugee Minor Foster Care program addresses the specific challenges and loss that each unaccompanied refugee minor faces. The services provided include individualized foster care, legal aid, healthcare, counseling, and case management. Our compassionate case managers provide trauma-informed care and help youth overcome language barriers while maintaining their cultural identity and cultural integration into their new home.
JOB SUMMARY: The Unaccompanied Refugee Minor (URM) Special Medical c oordinates services for refugee clients acting as liaison between refugee clients and community agencies.
ESSENTIAL DUTIES:
Assesses the medical and behavioral health needs of unaccompanied foster youth participating in the Unaccompanied Refugee Minor program (URM), ensuring and utilizing professional interpretation reflecting each youth’s unique language and culture. Makes recommendations to case workers regarding case activities. Ensures all URM documentation is maintained in an accurate and up to date fashion and that meets respective federal, state, and agency regulations are observed regarding confidentiality and case documentation management. Performs as liaison between URM and community agencies by maintaining good working relationships. Provides outreach and develops provider network for URM youth with medical/behavioral health needs. Assists URM in communication and may act as advocate to obtain access to treatment/or needed services. Works closely with other Unaccompanied Refugee Minor Program (URM) staff to assure appropriateness of service delivery and coordination of services delivered to URM with extensive medical/behavioral health needs; maintains ongoing communication, exchange of information; crisis intervention, conducts case staffing as needed. Provides recommendations for psychological care/or therapeutic placement; assisting the URM Program Therapist in determining behavioral health needs; judging which information is necessary to share with supervisor; communicating with behavioral health/trauma informed professionals to ensure client benefits from services. Maintains a caseload of URM youth/young adults with extensive medical/or behavioral health needs; submits relevant reports, documentation and service requests utilizing the DES AARPODS System, including but not limited to: (medical service requests), SIR (Significant Incident Reports) reports, and other relevant updates and documentation. Ensures youth receive appropriate and timely treatment based on their unique needs and case recommendations. Provides coaching to URM youth who are transitioning into the Semi-Independent Living (SIL) Program on topics such as health care system, health insurance, emotional well-being, etc.; ensures young adults are fully connected to supportive adult services after age 18; ensures youth are able to access services independently prior to program exit. Provides Technical Support to providers who are unfamiliar with the URM Population, including participating in panel discussions, trainings, and conferences. Provides coaching and support to foster parents regarding URM foster youth with medical/behavioral health needs, identifies ongoing training opportunities to foster parents in specific areas related to needs of foster youth. Assists Program Manager in conducting quarterly URM file reviews to ensure compliance with ORR requirements. Promote ongoing commitment to excellence and continual quality improvement to ensure the highest quality of services to our staff and clients. Performs other duties as assigned.
JOB REQUIREMENTS :
Education /Background : Minimum of a Bachelor’s degree with at least four (4) years of related professional and/or volunteer experience. One (1) year experience with unaccompanied/or immigrant children or youth in foster care system preferred. Bilingual Spanish speaker preferred. Job Knowledge : Must be able to relate well to a wide variety of groups and people in community. Must have good communication skills (verbal and written). Must be able to work under pressure and with varying deadlines. Must be sensitive to cultural differences within the agency and community. Fluent in Microsoft Office Applications (Word & Outlook). Working Conditions/Physical Demands : Must be able to work in a fast paced environment with moderate interruptions. Must be able to stoop, bend, squat, and ambulate over uneven floor surfaces including stairs and steps. Primarily office work, travel within area on a frequent basis, travel to other areas of state as needed. Evening and weekend hours required as needed. Must be able to visually and auditory assess and interact with clients to ensure their safety and wellbeing. M ay lift up to 25 pounds. Other requirements : Valid AZ driver’s license, reliable transportation and ability to show proof of registration/insurance. Level One Fingerprint clearance or ability to obtain fingerprint clearance; must maintain clearance. You must be 21 years of age or older to drive on behalf of Catholic Charities.
