Description

The position exists to manage clinical outcomes, cost, and customer satisfaction in the daily care assignment and in the operations of the unit. The incumbent assures care for patients and families reflective of the HealthNet organization values. This position coordinates the nursing and interdisciplinary team for daily. Additionally, this position exists to provide on-site management and clinical support to staff. The incumbent has responsibility to: provide direction and assistance with problem-solving; to ensure adequate staffing and material resources are available to support patient care; to support and facilitate operational activities such as provider team management, problem solving patient, family and physician complaints; to identify and address staff education deficits; and to communicate policy and procedure changes to staff on the shift for which they are assigned responsibility. Essential Functions Clinical Judgment: Conducts accurate clinical assessments; identifies and prioritizes patient/family strengths, needs and priorities; develops, implements then evaluates the patient's individualized plan of care; and modifies plan to meet mutually agreed-upon clinical outcomes in accordance with Patient Centered Medical Home (PCMH). Care is consistent with established policies, procedures and standards of care (follows algorithms, decision trees, and protocols in the delivery of patient care). Identifies the normal course of primary care in assigned patient population recognizing and intervening when deviations occur. Provides for and maintains a safe patient/family environment. Accurately communicates patient information and thoroughly documents nursing actions and plan of care. Manages own work responsibilities and will delegate patient care effectively based on clinical support needs and skills of team members. Seeks validation of clinical judgment from more experienced clinicians. Caring Practices: Shows respect for patients/family dignity and privacy in all patient/family interactions. Establishes and maintains mutual trust within the nurse-patient/family relationship. Corrects patient's service problems promptly and objectively. Listens and acknowledges patient's/family's knowledge of self and engages their participation in care. Provides for optimum comfort of patients and families utilizing standard evidence based approaches. Able to successfully use multiple forms of communication to appropriately provide patient/family support (presence, touch, eye contact, etc.). Supportive to new staff members and helps to facilitate their transition into primary care with HealthNet. Demonstrates self-care in order to preserve one's endurance as a care giver within a stressful environment. Response to Diversity: Seeks opportunities for individual growth in diversity training and responsiveness. Identifies, implements, then documents a plan of care that is sensitive to the unique needs of the patients and families. Aware of the usual cultural norms for key patient population seeking validation in individual situations. Provides age specific and developmentally appropriate patient/family care. Oversees the completion of teaching plan by evaluating then documenting patient/family learning. Clinical Practice: Assesses critical needs of patients who are at risk for complication, unstable and/or belong to services outside usual population of the unit. Consults with house staff and/or unit staff on operational course of action as needed (i.e., staffing, patient transfers for emergency, temporary support for clinicians/traveling) Provides support for procedures as needed. Serves as a resource to House Staff in resolving patient care or system issues at the designated site(s). Responds to all emergency management situations to support patient care as needed. Assists with resolution of patient/family concerns. Provides direct patient care as needed to maintain expert clinical skills. Supervision/Operations Support: Provides on-site supervision of Nursing/PCA staff to facilitate operations for the assigned clinical site(s). Makes rounds at designated site(s). Assist all units/staff with problem-solving to obtain necessary equipment, medications, and supplies on an as needed basis. Evaluates staffing plans for the assigned clinical site(s). Assists charge nurses with clinical assessments as needed. Evaluates/makes decisions about staffing plans and schedules. Communicates and problem solves with the Manager or Clinical Services as needed. Facilitates incident reporting and variance reports to appropriate management staff. Provides Manager of Clinical Services of assigned site(s) with management support as assigned (i. e. staffing/scheduling, in-service training, off-shift communication with staff). Identifies operational issues, staff performance problems and/or educational deficits on the designated shift and propose corrective action plans for resolution. As directed by leadership, may be assigned to complete employee Contribution Management summary for assigned staff. Implements staff development plans for individual employees. As directed by leadership, may be responsible for completing Level 1 Attendance Warnings and Documented Counseling Corrective Action. Participates in validation sessions as appropriate. Attends assigned leadership meetings at least monthly. Excellence Orientation: Takes actions to continually improve quality and safety in daily work. Actively participates in improving work processes to meet and exceed customer requirements. Is compliant with all clinical policy and procedure and understands the safety risks posed to patients when short cuts are taken. Demonstrates the ability to perform appropriate monitoring of patients associated with drug administration and take appropriate actions as necessary.
