We even offer pet insurance for your four-legged family members.  </li>\n</ul>\n<ul>\n <li>Early access to earned wages with Daily Pay, tuition reimbursement to help further your career and adoption assistance to help you grow your family.   </li>\n</ul>\n<p>With a collective goal to champion a culture of belonging where everyone feels valued and respected, we honor the ways people are unique and embrace what brings us together.  </p>\n<p>And, we believe equipping you with support and development opportunities is a vital part of delivering an exceptional employment experience. </p>\n<p>Find a fulfilling career and make a difference with UnityPoint Health. </p>\n<h2>Responsibilities</h2>\n<p>· Makes visits to the homes of patients requiring home health physical therapy services and assumes responsibility for an ongoing interdisciplinary assessment of the patient.</p>\n<p>· Performs initial and comprehensive assessments on all patients in accordance with agency policies and procedures.</p>\n<p>· Evaluates and assesses the patient’s level of functioning by applying appropriate tests.</p>\n<p>· Treats clients to relieve pain, develop and restore function and maintain maximum performance using appropriate physical therapy treatments, modalities and procedures as stated in the physical therapy plan of care.</p>\n<p>· Initiates diagnostic, preventative and rehabilitative procedures as appropriate to the patient’s care delivery</p>\n<h2>Qualifications</h2>\n<p><strong>Education:</strong></p>\n<ul>\n <li>Graduate of a Physical Therapist education program approved by The Commission on Accreditation in Physical Therapy Education (CAPTE), a successor organization of CAPTE, or an education program outside the United States determined to be substantially equivalent to physical therapist entry-level education in the United States by a credentials evaluation organization approved by the American Physical Therapy Association.</li>\n <li>Passed an examination for Physical Therapists approved by the state in which physical therapy services are provided.</li>\n</ul>\n<p><strong>Licensure/Certifications:</strong></p>\n<ul>\n <li>Licensed as a PT by the state in which practicing, unless licensure does not apply within state regulation.</li>\n <li>Valid licensed driver with automobile insurance in accordance with state(s) and/or organizational requirements.</li>\n <li>Mandatory Reporter: Child & Dependent Adult Abuse.</li>\n <li>CPR: Maintain a valid Basic Life Support (BLS) Healthcare Provider Card with Re-certification.</li>\n</ul>\n<p><strong>Minimum Experience:</strong></p>\n<ul>\n <li>One year experience caring for similar patient population as to be assigned.</li>\n</ul>\n<p><strong>Other:</strong></p>\n<ul>\n <li>Use of usual and customary equipment used to perform essential functions of the position.</li>\n</ul>\n<p><strong>Apply Today!</strong></p>\n<ul>\n <li>Ready to take the next step in your healthcare career?
