    "description_html": "<p>Professional Medical Practice, P.C., is looking for an <strong>Alcohol and Substance Abuse Counselor</strong> (CASAC, CASAC2) to work in the medical departments at multiple correctional facilities throughout New York State.</p><p><br></p><p>This is a Full-Time Float position (40 hours/week) with availability to cover the following facilities:</p><ul><li><strong>Fulton County Correctional Facility</strong>: 2710 State HWY 29, Johnstown, NY. 12095</li><li><strong>Montgomery County Jail</strong>: 200 Clark Dr, Fultonville, NY 12072</li><li><strong>Schoharie County Jail</strong>: 157 Steadman Way, Howes Cave, NY 12092</li><li><strong>Columbia County Jail:</strong> 85 Industrial Tract, Hudson, NY 12534</li><li><strong>Broome County Correctional Facility:</strong> 155 Lt. VanWinkle Drive, Binghamton, NY 13905</li><li><strong>Warren County Jail: </strong>1400 US-9, Lake George, NY 12845</li><li>Additional Facilities as needed.</li></ul><p>Hourly Rate: $27.00- 32.00. Offering a Vehicle Stipend in addition.</p><p>You must have a <strong>CASAC or CASAC2 </strong>to qualify for this role. CASAC-T's do <strong>not</strong> qualify.</p><p><br></p><p><u><strong>Job Responsibilities:</strong></u></p><ul><li>Responsible for the clinical monitoring, coordination, and implementation of the behavioral health program in accordance with Company policies and procedures.</li><li>Serve as the liaison between medical and facility staff as it pertains to Medication Assisted Treatment (MAT).</li><li>Engage patient in therapeutic interventions with the goal of addressing substance use related concerns.</li><li>Facilitate group and individual sessions with patients in the MAT program.</li><li>Actively assist with the detection and prevention of patient suicide.</li><li>Document all patient encounters in a timely manner utilizing the SOAPE note format.</li><li>Assist and/or collaborate with facility staff regarding discharge planning for patients in the MAT program, where applicable.</li><li>Facilitate/participate in interdisciplinary team meetings and/or case consultations with staff across disciplines.</li><li>Participate as a member of the Quality Assurance Committee and any sub-committees. Assist in the establishment of Quality Assurance standards and audit criteria.</li><li>Serve as a member of various committees within the medical unit and facility as necessary.</li></ul><p><u><strong>Benefits</strong></u><strong>:</strong></p><p>Professional Care believes in offering a competitive compensation and benefits package to all their employees!
