Your responsibilities will include:</p>\n<ul>\n <li>Assess patients' physical, social, spiritual, and psychological needs</li>\n <li>Develop, implement, and evaluate individualized care plans</li>\n <li>Incorporate teaching into care plans to educate patients and families</li>\n <li>Make referrals to resources as needed</li>\n <li>Support fellow healthcare professionals</li>\n <li>Operate within the framework of Providence Health Care's mission, vision, and values</li>\n</ul>\n<h2>Minimum Nurse-to-Patient Ratio</h2>Minimum Nurse-to-Patient Ratios (mNPR) set the minimum number of nurses required to safely care for patients on a given unit. Developed in partnership with the Ministry of Health, the BC Nurses' Union (BCNU), and health organizations across British Columbia, mNPR provides a consistent and transparent approach to staffing that supports safe nursing practice and high-quality patient care. The unit has 65 inpatient beds dedicated to supporting the specialized rehabilitation needs of older adults following a life-altering event, such as a stroke, other neurological issues, orthopedic trauma, as well as major surgeries like hip replacements and leg amputations. In addition, more than 15,000 patients visit HFH Rehab each year to receive outpatient rehabilitation services for needs resulting from illness or injury</p>\n<p><strong>State-of-the-art facilities</strong>: The new St. Paul’s Hospital and health campus will represent the most innovative approach to the delivery of integrated care in B.C. and Canada. It is designed to appropriately address the future health needs of patients, families, and our communities. From hospital care to primary and community health solutions, the new St. Paul’s Hospital and health campus will continue to lead innovations in care, research, and teaching. The new St. Paul’s Hospital is expected to open in 2027.</p>\n<p> </p>\n<p><strong></strong></p>\n<p> </p>\n<p><strong>Your Day to Day:</strong></p>\n<ul>\n <li>Conduct thorough patient/resident assessments through chart reviews, observations, interviews, and collaboration with peers</li>\n <li>Develop, implement, and evaluate nursing care plans based on evidence-based practices, prioritizing patient/resident needs</li>\n <li>Adhere to established policies, procedures, and PHC standards while providing high-quality nursing care</li>\n <li>Monitor patient/resident progress, evaluate outcomes, and adjust care plans as needed for optimal results</li>\n <li>Maintain accurate records of assessments, care plans, patient/resident status, and discharge plans using manual and computerized systems.</li>\n <li>Identify and address patient/resident and family learning needs, providing education and referrals as necessary</li>\n <li>Provide direction to staff, promote teamwork, and share clinical knowledge to enhance patient care</li>\n <li>Support nursing research initiatives and contribute to the improvement of patient/resident care policies and procedures</li>\n <li>Advocate for patients/residents' rights and dignity, facilitate discharge plans, and collaborate with families and community resources</li>\n <li>Perform additional duties as assigned to support the healthcare team</li>\n</ul>\n<p><strong>Who We Are:</strong><strong> </strong></p>\n<p><strong>Providence Health Care</strong> is one of the largest faith-based health care organizations in Canada.
