    "description": "<p>Green River Area Development District (GRADD) is seeking a dedicated<span> </span><span style=\"font-weight: bold\">Case Manager</span><span> </span>to provide support and coordination for individuals with physical disabilities through Kentucky’s<span> </span><span style=\"font-weight: bold\">Home and Community-Based (HCB) Waiver</span><span> </span>program. This role ensures clients receive the necessary resources to live as independently as possible within their communities.</p>\n<p><br></p>\n<p>The<span> </span><span style=\"font-weight: bold\">HCB Waiver</span><span> </span>is part of Kentucky’s<span> </span><span style=\"font-weight: bold\">1915(c) HCBS Medicaid waiver program</span>, allowing participants to hire their own employees for non-medical, non-residential services under<span> </span><span style=\"font-weight: bold\">Participant Directed Services (PDS)</span>.</p>\n<p><br></p>\n<p><span style=\"font-size: 16px\"><span style=\"font-weight: bold\">Key Responsibilities:</span></span></p>\n<ul>\n<li>Perform case management functions for the<span> </span><span style=\"font-weight: bold\">Home and Community-Based Waiver (HCB)</span><span> </span>program.</li>\n<li>Review client eligibility and coordinate reassessments to maintain Medicaid Waiver compliance.</li>\n<li>Complete and submit all required Medicaid waiver forms using the<span> </span><span style=\"font-weight: bold\">Medicaid Waiver Management Application (MWMA)</span>.</li>\n<li>Conduct initial and annual reassessments, including<span> </span><span style=\"font-weight: bold\">recertification and care plan development</span>.</li>\n<li>Maintain<span> </span><span style=\"font-weight: bold\">Participant Directed Services (PDS)</span><span> </span>employee and participant documentation per agency guidelines.</li>\n<li>Assist PDS employees and participants with necessary paperwork and guidance.</li>\n<li>Monitor<span> </span><span style=\"font-weight: bold\">member budgets, health, safety, and welfare</span><span> </span>through timesheet submission and compliance checks.</li>\n<li>Advocate for clients and families by connecting them with appropriate<span> </span><span style=\"font-weight: bold\">community resources</span>.</li>\n<li>Conduct<span> </span><span style=\"font-weight: bold\">monthly home visits</span><span> </span>to monitor services and track progress toward client goals.</li>\n<li>Collaborate with GRADD’s<span> </span><span style=\"font-weight: bold\">PDS Manager</span>, provider agencies,<span> </span><span style=\"font-weight: bold\">Department for Community Based Services</span>, and<span> </span><span style=\"font-weight: bold\">Department for Medicaid Services</span><span> </span>to meet client needs.</li>\n<li>Maintain accurate<span> </span><span style=\"font-weight: bold\">participant records, logs, and reports</span>.</li>\n<li>Refer waiver applicants to the<span> </span><span style=\"font-weight: bold\">Aging and Disability Resource Center staff</span><span> </span>in a timely manner.</li>\n<li>Attend<span> </span><span style=\"font-weight: bold\">staff meetings and professional development trainings</span>.</li>\n</ul>\n<p><br></p>\n<p><span style=\"font-size: 16px\"><span style=\"font-weight: bold\">Other Responsibilities:</span></span></p>\n<ul>\n<li>Assist with special projects as needed.</li>\n</ul>\n<p><br></p>\n<p><span style=\"font-size: 16px\"><span style=\"font-weight: bold\">Qualifications &amp; Requirements:</span></span></p>\n<ul>\n<li><span style=\"font-weight: bold\">Education:</span>\n<ul>\n<li>Bachelor’s degree in<span> </span><span style=\"font-weight: bold\">Social Work, Gerontology, Psychology, Sociology, or a related field</span><span> </span>with<span> </span><span style=\"font-weight: bold\">two (2) years</span><span> </span>of experience working with individuals with special needs.</li>\n<li>Bachelor’s degree in<span> </span><span style=\"font-weight: bold\">Nursing</span><span> </span>with a<span> </span><span style=\"font-weight: bold\">current Kentucky nursing license</span><span> </span>and<span> </span><span style=\"font-weight: bold\">two (2) years</span><span> </span>of experience working with special needs populations.</li>\n<li>Associate degree with<span> </span><span style=\"font-weight: bold\">two (2) years</span><span> </span>of experience working with special needs populations.</li>\n</ul>\n</li>\n<li><span style=\"font-weight: bold\">Experience:</span>\n<ul>\n<li>Reliable<span> </span><span style=\"font-weight: bold\">transportation</span><span> </span>required.</li>\n<li>Knowledge of<span> </span><span style=\"font-weight: bold\">special needs populations, community organizations, and service resources</span>.</li>\n</ul>\n</li>\n<li><span style=\"font-weight: bold\">Skills:</span>\n<ul>\n<li>Strong<span> </span><span style=\"font-weight: bold\">verbal and written communication</span><span> </span>skills.</li>\n<li>Excellent<span> </span><span style=\"font-weight: bold\">organizational skills</span><span> </span>and the ability to multitask.</li>\n<li>Proficiency in<span> </span><span style=\"font-weight: bold\">computer applications</span><span> </span>and documentation software.</li>\n</ul>\n</li>\n</ul>\n<p><br><br></p>\n<p><span style=\"font-size: 18px\"><span style=\"font-weight: bold\">Why Join Us?</span></span></p>\n<p>At GRADD, we offer a dynamic work environment where you can make a real impact on the community.
