    "description": "<p>GENERAL PURPOSE:&nbsp;</p>\r\n\r\n<p>The LVN performs selected nurse care under the direction of the registered nurse for patients of all ages in their place of residence, coordinate care with the interdisciplinary group, patient/family and referring agency.&nbsp;&nbsp; Ensure staff and other support resources are appropriately utilized and a high level of interdisciplinary team work is maintained through delegation of duties.&nbsp;&nbsp;</p>\r\n\r\n<p>QUALIFICATIONS:&nbsp;</p>\r\n\r\n<ul>\r\n\t<li>\r\n\t<p>Successful completion of a certified licensed practical/vocational nursing program.&nbsp;</p>\r\n\t</li>\r\n</ul>\r\n\r\n<ul>\r\n\t<li>\r\n\t<p>Licensed to practice as a practical/vocational nurse in the state.&nbsp;</p>\r\n\t</li>\r\n</ul>\r\n\r\n<ul>\r\n\t<li>\r\n\t<p>Minimum of six (6) months to one (1) year of clinical experience.&nbsp;</p>\r\n\t</li>\r\n</ul>\r\n\r\n<ul>\r\n\t<li>\r\n\t<p>Six (6) months to one (1) year of previous home health or hospice experience preferred&nbsp;</p>\r\n\t</li>\r\n</ul>\r\n\r\n<ul>\r\n\t<li>\r\n\t<p>Reliable transportation.
