You play a key role in ensuring caregivers&rsquo; voices are heard and that families are equipped to advocate for their children and navigate complex systems successfully.</p><hr><h3><strong>What You&rsquo;ll Do:</strong></h3><p>The Family Partner provides peer‑based support, advocacy, and guidance to caregivers of youth with significant emotional and behavioral needs, helping families build skills, confidence, and connections.</p><p>&bull; Provide guidance, coaching, and support to caregivers navigating their child&rsquo;s needs and services<br>&bull; Help caregivers articulate youth strengths, needs, and goals within the Care Planning Team (CPT)<br>&bull; Engage families in home and community‑based activities aligned with Individual Care Plans<br>&bull; Support caregivers in developing self‑care strategies and connecting to peer supports and resources<br>&bull; Attend meetings (IEPs, treatment teams, hospital discharges, etc.) and assist with transition planning<br>&bull; Participate in and, when appropriate, facilitate Care Planning Team meetings<br>&bull; Conduct weekly in‑person home visits and provide ongoing telephone support<br>&bull; Provide transportation to families as needed<br>&bull; Maintain professional boundaries and ethical standards at all times<br>&bull; Participate in weekly supervision (administrative, clinical, and group supervision)<br>&bull; Complete timely and accurate documentation including progress notes and treatment goals</p><hr><h3><strong>What You Bring:</strong></h3><h4><strong>Required Experience &amp; Lived Experience:</strong></h4><p>&bull; Must have lived experience as a parent/caregiver of a youth with special needs<br>&bull; Experience with youth with mental health needs <strong>strongly preferred</strong></p><h4><strong>Education &amp; Experience (one required):</strong></h4><p>&bull; High School Diploma or GED&nbsp;<strong>and</strong> minimum 2 years&rsquo; experience with children/adolescents/TAY<br>&bull; Associate degree in a human services field&nbsp;<strong>and</strong> 1 year relevant experience<br>&bull; Bachelor&rsquo;s degree in a human services field <strong>and</strong> 1 year relevant experience</p><h4><strong>Skills &amp; Qualifications:</strong></h4><p>&bull; Experience navigating child- and family-serving systems (schools, behavioral health, etc.)<br>&bull; Strong communication, organizational, and interpersonal skills<br>&bull; Ability to engage families without judgment and build trusting relationships<br>&bull; Knowledge of community resources and culturally responsive services<br>&bull; Ability to support families in crisis and high‑stress situations<br>&bull; Team‑oriented with the ability to work independently in the field</p><h4><strong>Additional Requirements:</strong></h4><p>&bull; Valid driver&rsquo;s license and reliable transportation required</p><br></div>\n<br/>Monday - Friday 8:00am - 4:00pm<br/>40",
