Description Midland Care Connection Midland Care Connection is a not-for-profit, community-based organization dedicated to helping older adults and seriously ill individuals stay independent and supported in their homes. Established in 1978, we offer a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and our signature PACE (Program of All-Inclusive Care for the Elderly) program. Our mission centers on "meeting individual needs through innovative care solutions," and we serve people across multiple counties in northeast Kansas. Community Home Health Aide Helps older adults remain safely and comfortably in their own homes. The job focuses on assisting clients with daily personal care needs such as bathing, dressing, grooming, using the restroom, eating, and moving around the home. Home health aides also provide companionship and emotional support, which is especially important for seniors who may live alone or have health challenges. In addition to personal care, aides often help with light housekeeping, laundry, meal preparation, and medication reminders. They follow a care plan created by nurses and other healthcare professionals and report any changes they notice in a client’s condition. The position is hands-on and physically active. Aides may help clients transfer from beds or chairs, assist with walking, and travel between clients’ homes during the workday.
