Job Title: Part-Time Bilingual Service Navigator - Spanish

Position Summary:
The Bilingual Service Navigator plays a crucial role in assisting clients and families—particularly those who face language barriers—in accessing and coordinating essential services. The Service Navigator is responsible for assessing client needs, developing individualized service plans, and connecting clients with community resources. A central focus of this role is building trusting relationships with non-English-speaking clients. Fluency in both English and Spanish is required to ensure effective communication and high-quality service delivery.
Duties and Responsibilities:
Service Coordination
Conduct client intake, interviews, and needs assessments. Develop and implement individualized service plans tailored to client goals. Coordinate access to housing, education, healthcare, employment, and social services. Partner with community agencies, healthcare providers, and other organizations to align supports. Bilingual Support
Provide verbal and written interpretation and translation services (English/Spanish). Translate forms, program materials, and other documents as needed. Serve as a language liaison during meetings, appointments, and community events. Advocacy
Advocate for clients within educational, healthcare, legal, and social service systems. Identify and address barriers to service access (e.g., language, transportation, system navigation). Empower clients with information, tools, and guidance to access services independently. Case Management & Follow-up
Monitor progress of service plans, adjusting supports as needed. Conduct follow-up to ensure services effectively meet client needs. Maintain accurate and timely records of interactions, services, and outcomes. Collaboration
Build strong partnerships with schools, government agencies, healthcare providers, and community organizations. Work collaboratively with internal teams to deliver comprehensive support. Attend meetings, workshops, and training to remain current on resources and best practices. Documentation & Reporting
Maintain confidential client records in compliance with regulations. Prepare and submit timely reports on client progress and outcomes. Utilize case management systems to document services and track data. Community Outreach
Conduct outreach to raise awareness of services among non-English-speaking populations. Engage community members to identify individuals and families in need. Represent the program at community events, resource fairs, and information sessions.
Qualifications:
Education and Experience
Bachelor’s or Associate’s degree in Social Work, Human Services, Public Health, or related field. Equivalent relevant experience may be considered. Minimum of 2 years of experience in service coordination, case management, or a related role, preferably with diverse and multilingual populations. Language Skills
Professional fluency in English and Spanish (speaking, reading, and writing) required. Knowledge, Skills, and Abilities
Strong knowledge of local, state, and federal community resources. Excellent interpersonal, communication, and relationship-building skills. Ability to manage multiple cases, tasks, and deadlines simultaneously. Cultural competence and sensitivity in working with diverse populations. Proficiency with Microsoft Office; experience with case management software preferred.
Additional Requirements:
Current Act 114, Act 34, Act 151 and NSOR Clearances required prior to an offer (Must be dated within 1 year of hire date).
School Personnel Health Record (Form H511.340) must be completed and received by Human Resources prior to hire date.
Valid driver’s license and reliable transportation for home visits and community meetings.
Flexibility to work some evenings or weekends as needed.
