Job Summary:
The Personal Support Worker (PSW) is responsible for rendering a range of services for the clients of the program consistent with its basic values and goals. The PSW will support, as assigned, the needs of the vulnerable, at risk, older adults and seniors living in the multi-sites under the auspices of the program. This support relates to the physical, cognitive, mental health and social/recreational needs of the clients. The staff member will maintain the established routines of the program including all security, fire and general health and safety practices. The PSW will perform these and other delegated responsibilities to ensure the quality of personal care of the clients of the program.
Social Medicine Housing Solution (Social Medicine) is an interdisciplinary team of providers from the University Health Network and LOFT Community Services that provides wrap around services to individuals with concurrent conditions that span physical and mental health challenges and social and economic vulnerability who are experiencing homelessness or at risk of losing their housing.
Social Medicine delivers clinical hands-on support to those with complex healthcare needs providing outreach and case management services with access to psychiatric and medical assessment, consultation, and access to treatment, service planning, advocacy, and system navigation support. The aim of this partnership is to provide low barrier access to housing for these individuals, while assisting in stabilizing their physical and mental health, substance use, behavioural, psychosocial, and social drivers of health challenges. The Social Medicine team works from a housing first and harm reduction approach. The goal is to work with individuals in getting connected to housing and follow up support that meets the level of need required for each individual to live successfully in the community.
What You Will Do: Client Care & Support: Provide compassionate support for clients’ physical and mental health needs, including medication reminders, health monitoring, and coordination with healthcare professionals. Activities of Daily Living (ADLs): Assist with personal care, hygiene, mobility, meal preparation, and maintaining a clean, safe, and comfortable living environment. Client Engagement & Empowerment: Build respectful relationships that promote dignity, independence, and active participation in daily, social, and recreational activities. Behavioral & Emotional Support: Assist clients in managing behavioural and cognitive challenges, offering guidance, reassurance, and companionship. Safety & Emergency Response: Ensure a safe environment by following health and safety procedures, responding to emergencies, and reporting incidents appropriately. Medication & Clinical Support: Follow established medication protocols, support self-administration programs, and collaborate with healthcare providers to ensure safe practices. Documentation & Team Collaboration: Maintain accurate care records, participate in care planning, and communicate effectively with the interdisciplinary team. Program & Operational Support: Assist with program activities, transportation for client outings, and day-to-day operations, including basic administrative tasks when required. Flexibility & Shift Coverage: Work rotating shifts, provide coverage as needed, and support overall program operations, including during the absence of senior staff. Additional responsibilities may be assigned by the Program Manager and/or designated supervisor to support overall program operations and continuity of care. What You Bring: Post-secondary education in Human Services or a related field (e.g., PSW Certificate, Gerontology Diploma). Experience supporting seniors and vulnerable populations, including individuals with cognitive and mental health needs. Knowledge of medication administration, health conditions, and psychiatric care. Current First Aid and CPR certification. Experience coordinating social and recreational activities is an asset. Strong skills in assessment, problem-solving, and crisis intervention. Ability to engage clients in a positive, respectful, and supportive manner. Ability to work collaboratively within a multidisciplinary team. Effective verbal and written communication skills. Demonstrated creativity, flexibility, and adaptability. Proficiency with basic computer applications. A valid Ontario Driver’s License is preferred. Experience in social/recreational co-ordination preferred. Work Environment: Expected to meet people outside, making exposure to the weather an expectation of the position. A car is not a requirement of the position however driving for prolonged periods of time with frequent stops is an essential part of the job. You may need to walk on rough terrain from time-to-time, for example uneven ground, parks, or ravines while providing outreach to community members sleeping outside. The aim of this partnership is to provide low barrier access to housing for these individuals, while assisting in stabilizing their physical and mental health, substance use, behavioural, psychosocial, and social drivers of health challenges. The Social Medicine team works from a housing first and harm reduction approach.
