Primary Care Access Clinician
THE ORGANIZATION
Outer Cape Health Services (OCHS) provides a full range of primary health care and supportive social services that promote the health and well-being of all who live in or visit the ten outermost towns of Cape Cod. With locations in Harwich Port, Wellfleet and Provincetown, OCHS cares for more than 20,000 patients annually, and no one is denied access to services due to an inability to pay.

Primary Purpose of Position:
As a Patient-Centered Medical Home (PCMH), we are committed to providing accessible, high-quality, team-based care. This position is designed for an on-site clinician who functions as the "physical anchor" for a shared patient panel, working in a dyad leadership model with a remote Primary Care Clinician.
Reporting to the Site Medical Director, the Access Clinician serves as the on-site clinical lead for a designated patient panel. Working in a formal dyad partnership with a Remote PCP, this role focuses on providing in-person clinical exams, urgent care interventions, and procedural support that cannot be performed via telehealth, ensuring seamless continuity of care.
Essential Functions:
Dyad Collaboration: Partner closely with a Remote PCP to co-manage a patient panel, serving as the on-site provider for physical assessments, diagnostic tests, and hands-on clinical needs.
On-Site Clinical Leadership: Lead the local care team (RNs, MAs, Patient Access staff) to coordinate daily clinical operations and patient flow.
Direct Patient Care: Maintain high-access availability for the shared panel, providing same-day appointments, urgent care needs, and flexible scheduling to meet patient demand.
Collaborative Huddles: Lead daily on-site team huddles (bridging in the Remote PCP virtually) to anticipate patient needs and identify care gaps.
Mentorship & Oversight: Provide clinical oversight and support to nursing and allied health staff to ensure team members work at the top of their licensure.
Integrated Care: Work in-person with Integrated Behavioral Health providers and Community Health Workers to address social determinants of health and mental health needs.
Performance Improvement: Participate in patient care audits, population health activities, and quality initiatives to improve clinical outcomes.
Administrative Duties: Complete clinical documentation in a timely manner consistent with OCHS standards and participate in provider meetings as directed.
Coverage Responsibilities: Participate in on-call responsibilities, weekend coverage, and skilled nursing facility coverage as assigned.
Other related duties and projects as assigned.
Qualifications
Licensure: Practice within the scope of licensure and certification consistent with Massachusetts law.
Physician Applicants: M.D. or D.O., board certification or eligibility in an appropriate specialty.
NP/PA Applicants: Graduation from an accredited program and current certification.
Mandatory Credentials: Valid and unrestricted license to practice in the Commonwealth of Massachusetts, BLS certification, and valid DEA registration.
Program Eligibility: Eligible to participate in Medicare, Medicaid, and other third-party payer programs.
Team Orientation: Excellent interpersonal skills, communication skills, organizational abilities, computer experience, and the ability to work within a collaborative team setting with a commitment to the PCMH model.
Cultural Competence: Ability to work effectively in a culturally diverse environment and serve a seasonal, community-based population.
Excellent written and verbal communication skills – bi-lingual helpful.
Proficiency in computer skills including EPIC, Outlook, Microsoft office, OneDrive, etc.
Ability to adhere to strict confidentiality standards.
Demonstrated ability to work effectively both as a team member and independently.
