They coordinate care with the Interdisciplinary Group/Team, patient/family, and referring agency, and assume responsibility for coordination of care.&nbsp;</p><p><br></p><p><strong>Role Expectations:</strong></p><ul><li>Assess hospice patients utilizing the Nursing Assessment and LCD criteria to identify their physical, psycho-social, and environmental needs.</li><li>Develop and document the plan of care in correlation with the Assessment</li><li>Provides nursing care utilizing infection control measures (OSHA)</li><li>Work with Interdisciplinary Group/Team to ensure patient/family needs are met.</li><li>Communicate with attending physicians, hospice physicians, and other physicians involved in the patient’s care.</li><li>Timely submission of all required paperwork.</li><li>Demonstrates commitment and professional growth by participating in hospice-sponsored in-service programs and maintaining or improving competency.
