That is where we step in.</p><p>This position provides basic social services to individuals and/or families participating in the Healthy Start Program in Lake County. &nbsp;The responsibilities include case management for Healthy Start clients and education and ensuring clients receive appropriate referrals and services in compliance with Healthy Start Guidelines. Funding for this position is provided by the Healthy Start contract to improve pregnancy outcomes.&nbsp;</p><p><strong>ESSENTIAL FUNCTIONS:</strong></p><ul type=\"disc\"><li>Perform case management services for individuals/families receiving Healthy Start services. &nbsp;</li><li>Assist clients in accessing community services and provides information and education to clients about smoking cessation, childbirth education, breastfeeding support, parenting and infant growth and development.&nbsp;</li><li>Initiate tracking procedures for client, family of client and client&rsquo;s provider to manage appointments and referrals for services. &nbsp;</li><li>Maintain/update client files, tickler cards and ensure Healthy Start database is updated.&nbsp;</li><li>Assess the educational needs of Healthy Start clients who are at risk for poor pregnancy outcomes.</li><li>Assist with providing office coverage and be available to receive any phone calls or assist any visitors to the Healthy Start Office during required office hours.</li><li>Complete all Healthy Start assessments and home visits as required per program guidelines.</li><li>Other duties as assigned.&nbsp;</li></ul><p><strong>OTHER FUNCTIONS:</strong></p><p>This list of functions is not intended to be exhaustive. Kids Central reserves the right to revise this job description as needed to comply with actual job requirements.</p><p><strong>QUALIFICATIONS:</strong></p><ul type=\"disc\"><li><u>Education/Experience</u></li></ul><p>&nbsp; &nbsp; &nbsp; &nbsp; &nbsp; A bachelor&rsquo;s degree in social sciences, Social Work, Human Services, or a Health-Related Field such as Health Education, Child and Maternal Health, or Public Health is required. 2 years of related experience is preferred.</p><p>&nbsp; &nbsp; &nbsp; &nbsp; &nbsp; Experience conducting home visits with a similar population is preferred.&nbsp;</p><ul type=\"disc\"><li><u>Licensure/Certifications</u></li></ul><p>&nbsp; &nbsp; &nbsp; &nbsp; &nbsp; Valid Florida driver&#39;s license and insurable by the agency&rsquo;s insurance carrier.&nbsp;</p><ul type=\"disc\"><li><u>Other Requirements</u></li></ul><p>&nbsp; &nbsp; &nbsp; &nbsp; &nbsp; Must successfully clear Level II Background Screen.</p><p>&nbsp; &nbsp; &nbsp; &nbsp; &nbsp; Must speak fluent Spanish</p><p>&nbsp; &nbsp; &nbsp; &nbsp; &nbsp; Must possess an acceptable driving record and have reliable transportation.</p><p><strong>KNOWLEDGE, SKILLS &amp; ABILITIES:</strong></p><ul type=\"disc\"><li>Excellent knowledge of community resources</li><li>Excellent interpersonal and communication skills (verbal and written)</li><li>Excellent computer skills in Microsoft Office Suite (Word, Excel, PowerPoint)</li><li>Effective public speaking skills</li><li>Ability to travel up to 75%</li><li id=\"isPasted\">Ability to adapt to a hybrid work environment as needed &nbsp;&nbsp;</li><li>Ability to lift 10-25 pounds</li><li>Ability to safely operate a motor vehicle</li><li>Ability to work evenings and weekends as needed</li><li>Ability to work in an environment with frequent interruptions and a moderate noise level</li><li>Ability to independently complete job duties within required time frames</li><li>Knowledge of family and systems theory&nbsp;</li><li>Knowledge of obstetrics, pediatric health, and health care systems preferred</li></ul><p><strong>WORK ENVIORMENT/PHYSICAL DEMANDS:</strong></p><ul type=\"disc\"><li>The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. &nbsp;</li><li>While performing the duties of this job, the employee is generally in an office environment but may also be required to visit other settings such as a client&rsquo;s home, court, or other agencies.
