Job Title: Case Manager Department: Social Services Reports To: Social Services Director Classification: Non-Exempt Status: Full-Time, Regular with Benefits Salary Range: $70,000 - $80,000 Annually DOE Location: Onsite – Remote or hybrid work arrangements are not permitted Job postings remain open for up to thirty (30) days or until a qualified candidate is selected. JOB SUMMARY The Case Manager provides support services and resource coordination for youth, adults, elders, and families in need of social services assistance. This position assists individuals experiencing homelessness, housing instability, behavioral health challenges, financial hardship, or other barriers by connecting them with housing resources, rehabilitation programs, healthcare services, and community support programs. The Case Manager works directly with participants to assess needs, develop individualized service plans, coordinate supportive services, and promote long-term housing and life stability through a trauma-informed and culturally respectful approach. ESSENTIAL DUTIES Conduct initial intake interviews to determine program eligibility and service needs. Complete comprehensive assessments and intake documentation to identify appropriate housing options, supportive services, and case management needs. Develop individualized case plans with measurable goals and objectives. Conduct ongoing assessments and quarterly reviews to evaluate participant progress and update service plans as needed. Assist participants in developing housing stability plans and personal budgets. Support participants in obtaining necessary documents such as identification cards, Social Security cards, birth certificates, and other required records. Assist participants with securing or increasing income through employment resources, public assistance programs, and mainstream benefits. Coordinate with outside agencies, healthcare providers, legal advocates, behavioral health providers, housing agencies, and community organizations to support participant needs. Maintain accurate and timely case notes, documentation, hard files, and electronic records. Ensure confidentiality and compliance with applicable laws, policies, and program requirements. Participate in community meetings, outreach activities, advocacy efforts, and collaborative service initiatives as assigned. Model the mission, values, and professional standards of the Picayune Rancheria of the Chukchansi Indians in all interactions. Perform other duties as assigned. MINIMUM QUALIFICATIONS High School Diploma or GED required. Two (2) years of experience in social services, human services, healthcare, behavioral health, housing services, or a related field required. Ability to communicate professionally, empathetically, and effectively with individuals from diverse backgrounds. Strong interpersonal, organizational, and documentation skills. Ability to manage multiple cases and priorities in a fast-paced environment. Proficient in basic computer applications, data entry, and electronic file management. Ability to maintain confidentiality and handle sensitive information appropriately. Valid Driver’s License required. Must successfully pass a background investigation and pre-employment drug/alcohol screening. PREFERRED QUALIFICATIONS Bachelor’s Degree in Social Work, Human Services, Psychology, or a related field preferred. Previous case management experience preferred. Experience working with individuals or families experiencing homelessness, housing instability, mental health challenges, substance abuse issues, or other complex social service needs preferred. Experience working in trauma-informed or harm-reduction environments preferred. Knowledge of trauma-informed care, housing-first principles, and community resource systems preferred. Familiarity with Tribal culture, community resources, and Tribal government operations preferred. Ability to work independently and collaboratively with internal departments and external agencies preferred. During the interview process, PRCI Tribal Members will receive an additional 7.5 points (10% of the 75-point interview rubric). Non-PRCI Native American candidates will receive an additional 5 points (6.7% of the total points) in accordance with Indian Preference guidelines. Indian Preference Statement:  In compliance with 25 CFR Part 276 and Title VII of the Civil Rights Act, Sections 701(b) and 703(i), employment preference shall be given to qualified applicants who are enrolled members of the Picayune Rancheria of the Chukchansi Indians, and secondarily, to another qualified American Indian/Alaska Native Candidate.
