Join a team where compassion and ambition go hand-in-hand!</p>\n<p> </p>\n<p>✨ <strong>Benefits include:</strong>✅ Competitive compensation & benefits ✅ Medical, dental, vision✅ Company-paid life insurance ✅ Flexible spending accounts ✅ 403(b) retirement plan </p>\n<h3>Why VCC?</h3>\n<p>• 🏅 Winner of the <strong>2025 HRSA Gold Medal</strong> for Outstanding Care, placing VCC among the top 10% of Federally Qualified Health Centers in the U.S.</p>\n<p>• Recognized by HRSA as a National Quality Leader in Behavioral Health and Diabetes and for excellence in Preventive Health and Health IT.</p>\n<p>• A robust training & development culture to help you grow and advance your career.</p>\n<p>• A workplace built on respect, collaboration and passion for care.</p>\n<h2>Responsibilities</h2>\n<p>Provide intensive case management and care coordination to program participants and assist in program activities and administrative support.• Monitor participants, utilization of care with VCC and other agencies• Document attempted contacts, client visits and medical assessment information in chart following primary care visit.• Participate in weekly case conference meetings with interdisciplinary team• Enroll clients referred by provider into the program • Monitor participant’s utilization of care with the Clinic and other agencies • Encourage clients to attend program’s support groups and classes • Document attempted contacts, actual contacts and summary of services in case notes • Participate in weekly and monthly staff meetings or as needed • Complete all monthly activity reports at the end of each month • May organize and present health education classes • Coordinate appointments between clients and staff members• Enhance professional growth and development through participation in educational programs, current literature review, in-service meetings and workshops</p>\n<h2>Qualifications</h2>\n<p><strong><u>Minimum</u></strong></p>\n<ul>\n <li><p>High school graduate or equivalent</p></li>\n <li><p>Five years' case management experience or equivalent combination of education and experience</p></li>\n <li><p><strong>Bilingual English/Spanish</strong></p></li>\n <li><p>Valid California driver's license and vehicle insurance; reliable transportation, to travel for meetings and/or to gather information for reporting purposes; minimum two years' driving experience; no more than 2 violations or a single ticket/accident value at more than 1 point on driving record</p></li>\n <li><p>Availability and willingness to work evening and weekend hours, as required</p></li>\n</ul>\n<p><strong><u>Required Skills/Knowledge/Abilities</u></strong></p>\n<ul>\n <li>Knowledge of co-occuring conditions</li>\n <li>Effective written/verbal communication skills</li>\n <li>Familiarity with local HIV/AIDS and non-HIV specific resources for health and social service needs</li>\n <li>Familiarity with health and social service needs of target populations served</li>\n <li>Basic computer knowledge, internet use, MS Office and data entry skills</li>\n</ul>\n<p><strong>Pay Range</strong></p>\n<ul>\n <li>$24.00 - $26.00</li>\n</ul>",
