To qualify, patients must be unhoused, have an acute medical condition, and be medically and physically stable. This respite program offers medical care, case management, personal care, three meals per day, access to long-term housing resources and placement, and reunification services for clients who have lost contact with their family or support network.&nbsp;</span></p><p><span style=\"font-family: tahoma, sans-serif; color: rgb(0, 0, 0); font-size: 14px;\"><strong><u>Position Purpose</u></strong><u>:</u></span></p><p><span style=\"font-family: tahoma, sans-serif; color: rgb(0, 0, 0); font-size: 14px;\">Within the regulatory scope of practice and under the direction of the Program Director, the LVN assists in the provision of direct patient care by performing routine patient care procedures, technical and supportive functions in the recuperative care setting. <strong>&nbsp;</strong></span></p><p><span style=\"font-family: tahoma, sans-serif; color: rgb(0, 0, 0); font-size: 14px;\"><strong><u>Principal Responsibilities:</u></strong></span></p><ol start=\"1\" type=\"1\"><li style=\"font-family: tahoma, sans-serif; color: rgb(0, 0, 0); font-size: 14px;\">Performs routine patient care functions as prescribed by established policies and procedures within defined scope of education, training and responsibilities&nbsp;</li><li style=\"font-family: tahoma, sans-serif; color: rgb(0, 0, 0); font-size: 14px;\">Uses and practices &ldquo;basic assessment&rdquo; (e.g. data collection) and documents data in accordance with the policies and procedures of JWCH and the standards of LVN practice. &nbsp;</li><li style=\"font-family: tahoma, sans-serif; color: rgb(0, 0, 0); font-size: 14px;\">Contributes to evaluation of the patient&rsquo;s individualized interventions related to the care plan and treatment plan.&nbsp;</li><li style=\"font-family: tahoma, sans-serif; color: rgb(0, 0, 0); font-size: 14px;\">Performs basic patient care consistent with the policies and procedures of JWCH and the LVN licensure laws of the State of California. &nbsp;</li><li style=\"font-family: tahoma, sans-serif; color: rgb(0, 0, 0); font-size: 14px;\">Applies principles of aseptic technique and infection control per policy and procedures.&nbsp;</li><li style=\"font-family: tahoma, sans-serif; color: rgb(0, 0, 0); font-size: 14px;\">Consistently uses and reinforces proper body mechanics and appropriate personal protection equipment.&nbsp;</li><li style=\"font-family: tahoma, sans-serif; color: rgb(0, 0, 0); font-size: 14px;\">Recognizes the need for emergency care; initiates appropriate procedures within the scope of practice and alerts the Nurse Manager and physician in a timely and appropriate manner.&nbsp;</li><li style=\"font-family: tahoma, sans-serif; color: rgb(0, 0, 0); font-size: 14px;\">Conducts patient education and supports patient&rsquo;s self-management goals according to JWCH policies and procedures.&nbsp;</li><li style=\"font-family: tahoma, sans-serif; color: rgb(0, 0, 0); font-size: 14px;\">Delegates selected tasks to a medical assistant, consistent with that individuals job description and the polices and procedures of JWCH.&nbsp;</li><li style=\"font-family: tahoma, sans-serif; color: rgb(0, 0, 0); font-size: 14px;\">Assists medical personnel with procedures and/or diagnostic exams.</li><li style=\"font-family: tahoma, sans-serif; color: rgb(0, 0, 0); font-size: 14px;\">Keeps patient exam rooms and work areas clean and stocked. &nbsp;</li><li style=\"font-family: tahoma, sans-serif;
