Physical, physician notes, diagnostic testing, lab tests and other relevant information.</span> &nbsp;</p><p><span style=\"font-size: 12pt;\">Provides discharge planning for patients within the hospital setting and initiates</span> <span style=\"font-size: 12pt;\">prescriptions, medical equipment, staff visits and crisis care as warranted.</span> &nbsp;</p><p><span style=\"font-size: 12pt;\">Meets the needs of the newly referred patient/families, completing the admissions</span> <span style=\"font-size: 12pt;\">process as soon as possible after referral; within 24 hours or sooner.</span> &nbsp;</p><p><span style=\"font-size: 12pt;\">Schedules assessment visit with patient/family, coordinating with the admissions&nbsp;</span> <span style=\"font-size: 12pt;\">social worker when necessary.</span> &nbsp;</p><p><span style=\"font-size: 12pt;\">Gives a comprehensive explanation of hospice services and benefits addressing all</span> <span style=\"font-size: 12pt;\">components of care.</span> &nbsp;</p><p><span style=\"font-size: 12pt;\">Obtains signatures on all necessary consents ensuring that patient/families are</span> <span style=\"font-size: 12pt;\">informed of their rights and responsibilities.</span> &nbsp;</p><p><span style=\"font-size: 12pt;\">Performs a physical/environmental assessment, in addition to a psycho-social and&nbsp;</span> <span style=\"font-size: 12pt;\">spiritual assessment if an admissions social worker is not involved.</span> &nbsp;</p><p><span style=\"font-size: 12pt;\">Provides primary clinical care and teaching, as needed, to stabilize the patient and</span> <span style=\"font-size: 12pt;\">facilitate the admissions process.</span> &nbsp;</p><p><span style=\"font-size: 12pt;\">Documents assessments and establishes the written initial plan of care and plan of</span> <span style=\"font-size: 12pt;\">treatment.</span> &nbsp;</p><p>&nbsp;<span style=\"font-size: 12pt;\"><strong><ins>Qualifications</ins></strong></span> &nbsp;</p><ul><li><a href=\"https://workforcenow.adp.com/theme/admin.html\" target=\"_self\">&nbsp;<span style=\"font-size: 12pt;\">Florida R.N. license required;
