Direct service is provided in context of individual, family and group treatment modalities. Case coordination is facilitated by communication and collaboration with multidisciplined staff members, the patient, collaterals (family, school, legal, case managers), and managed care organizations across the continuum of care. The focus of case coordination is to ensure safe, effective, solution focused treatment at the least restrictive level of care possible.</p><p><strong>Job Responsibility</strong></p><p>1. Uses clinical knowledge of psychosocial dynamics and interviewing techniques to gather psychosocial history information, define presenting problem(s), assess strengths and liabilities, and formulate discharge goals.&nbsp;</p><p>2.Coordinates development of individualized interdisciplinary treatment plans.&nbsp;</p><p>3.Coordinates the activities and input of the treatment team members, community resources, and payers to meet the treatment needs of the patients (and families.)&nbsp;</p><p>4.Ensures appropriate resource utilization via utilization review process and conducts timely and efficient concurrent reviews with insurance companies if required&nbsp;</p><p>5.Facilitates contact with families and relevant collateral resources to maximize their involvement and understanding of treatment goals/discharge criteria.&nbsp;</p><p>6.Provides individual, family and group treatment.&nbsp;</p><p>7.Actively participates in treatment meetings.&nbsp;</p><p>8.Documents assessments, interventions, and discharge-planning process in a clear, concise, and timely manner.&nbsp;</p><p>9.Evaluates patient response to treatment.&nbsp;</p><p>10.Coordinates discharge plan with patient, family, treatment team, case managers, outpatient providers, payer source.&nbsp;</p><p>11.Develops and maintains cooperative working relationships with community health, welfare and social agencies, and reaches out to the community to develop new resources.&nbsp;</p><p>12.Assesses, integrates and appropriately uses all information provided by both department and outside contacts which may affect the delivery of services in the health care setting, including new regulations, legislation and clinical data.&nbsp;</p><p>13.Contributes to service and hospital program improvement by serving regularly and productively on department, service and hospital committees.&nbsp;</p><p>14.Assists with performance improvement activities and program development.&nbsp;</p><p>15.Performs related duties, as required.&nbsp;</p><p>*ADA Essential Functions</p><p>&nbsp;</p><p><strong>Job Qualification</strong></p><ul style=\"list-style-type: disc;\"><li>Master's Degree in Social Work, Mental Health Counseling, or Marriage and Family Therapy&nbsp;</li><li>Outpatient Program: LMSW or LMHC or LMFT required.&nbsp;</li><li>Inpatient/PHP: LMSW, LMFT, LMHC limited permit (Licensed Mental Health Counselor) preferred.&nbsp;</li><li>One year experience with case coordination preferred.&nbsp;<br>&nbsp;</li></ul><p><strong>*Additional Salary Detail</strong><br>The salary range and/or hourly rate listed is a good faith determination of potential base compensation that may be offered to a successful applicant for this position at the time of this job advertisement and may be modified in the future.
