    "ExternalDescriptionStr": "<p><strong>POSITION SUMMARY:</strong><br>\n St. Croix County Public Health is seeking a qualified nursing professional to provide public health nursing services, education, disease prevention, and community health support services to individuals, families, and populations within the community.</p>\n<p>Depending on qualifications, education, licensure, and experience, the selected candidate may be hired as either a Public Health Nurse or Registered Nurse – Public Health.</p>\n<p><strong>ESSENTIAL FUNCTIONS:</strong><br><i>The following duties are normal for this position. These are not to be construed as exclusive or all-inclusive. To perform this job successfully, an individual must be able to perform each duty satisfactorily. Other duties may be required and assigned.</i></p>\n<p><i><strong>Core Duties – Performed by Both Positions</strong></i></p>\n<ul>\n <li>Maintains current knowledge on medical information that influences services provided to clients.</li>\n <li>Assists with and/or manages communicable disease cases and outbreaks; investigates and educates regarding prevention and control in accordance with state mandates and statutes and maintains WEDSS (Wisconsin Electronic Disease Surveillance System) documentation.</li>\n <li>Assists in planning, preparing, and responding to Public Health preparedness events, including mass clinics.</li>\n <li>Provides direct client services following current department, state and federal guidelines and policies: Assesses, educates and evaluates client needs (ie: pregnant and postpartum women, infants, reproductive health clients, tuberculosis program clients, immunization clients, general public health clients) via home visits, clinic/office visits, or phone calls; develops nursing care plans and coordinates care.&nbsp;</li>\n <li>Assesses, educates, and evaluates clients regarding acute and chronic health conditions, psychosocial, and environmental needs and refers to appropriate resources.&nbsp;</li>\n <li>Prepares, updates, and maintains client records, utilizing various electronic health record systems.&nbsp;</li>\n <li>Assists in organizing, managing, updating, and implementing various Public Health programs and contracts. Coordinate projects to assure goals are addressed as assigned; Collect data and assist in analysis and outcome evaluation throughout all public health services and activities as appropriate.</li>\n <li>Assists in the writing of grant proposals, and oversees grant activities, reporting requirements, contractual obligations, and accreditation performance standards/requirements.</li>\n <li>Supports and participates in re-accreditation, 140 review, strategic planning, the performance management system including quality improvement, workforce development, emergency preparedness, community health needs assessment, and community health improvement planning as assigned.</li>\n <li>Provide appropriate health promotion, disease and injury prevention education in various formats to diverse audiences, including schools, media, organizations, and the public.&nbsp;</li>\n <li>Facilitates and manages community coalitions, networks and partnerships through outreach and collaboration.&nbsp;</li>\n <li>Works cooperatively and communicates effectively with local, county, state and federal partners, and inter-agency professional teams.</li>\n <li>Comprehensively assess needs related to public inquiries and provide appropriate education and referrals according to client needs</li>\n <li>Other duties as assigned.</li>\n</ul>\n<p><strong>Additional Duties – Public Health Nurse (BSN Level)</strong><br>\n The following duties may be assigned to employees hired into the Public Health Nurse classification:</p>\n<ul>\n <li>Provides leadership in planning, preparing, and responding to Public Health preparedness events, including mass clinics.&nbsp;</li>\n <li>Organizes, manages, updates, and oversees various Public Health programs and contracts with local, state and federal partners.&nbsp;</li>\n <li>Provides leadership in communicable disease response activities, outbreak management, and public health program and policy implementation and assessment.&nbsp;</li>\n <li>Oversees grant activities, reporting requirements, contractual obligations, and accreditation performance standards/requirements. Supervises the health care services provided by LPN’s, CNA’s, CMA’s, and support staff related to patient care and community programs.&nbsp;</li>\n <li>Provides oversight of clinical educational experiences for students.&nbsp;</li>\n <li>May assist with planning, implementing, and evaluating environmental health programs.</li>\n</ul>\n<p><strong>KNOWLEDGE, SKILLS AND ABILITIES &nbsp;</strong></p>\n<ul>\n <li>Ability to coordinate or support public health programs, community initiatives, emergency preparedness activities, and population-based health interventions consistent with assigned classification and qualifications.</li>\n <li>Knowledge of the principles, standards, practices and techniques of professional and public health nursing.</li>\n <li>Knowledge of the inherent dangers and precautions to be taken in using drugs and other medication.</li>\n <li>Knowledge of treatment, assessment, immunization and related medical services for patients in area of assignment.</li>\n <li>Knowledge of local and state rules, regulations, procedures and policies pertaining to nursing services.</li>\n <li>Knowledge, skills, and abilities regarding health equity, social determinates of health, public health ethics, and the public health core competencies.</li>\n <li>Knowledge of NIMS and ICS protocols.</li>\n <li>Knowledge of CLIA and OSHA.</li>\n <li>Knowledge of electronic records systems.</li>\n <li>Ability to perform detailed work accurately and independently in compliance with stringent time limits with appropriate direction and supervision.</li>\n <li>Ability to prioritize duties to accomplish a high volume of work product while adapting to constant changes in priority.</li>\n <li>Ability to type accurately at a reasonable rate of speed.</li>\n <li>Knowledge of County policies, procedures, practices, and departmental operating requirements.</li>\n <li>Ability to establish and maintain effective working relationships with others.</li>\n <li>Ability to utilize Microsoft 365 applications at a professional level including: Microsoft Word, Excel, Outlook, OneDrive, Teams.&nbsp;</li>\n <li>Ability to work the allocated hours of the position.</li>\n</ul>\n<p><strong>LANGUAGE SKILLS &nbsp;</strong></p>\n<ul>\n <li>Ability to communicate effectively with clients, families, community partners, healthcare providers, and multidisciplinary teams.</li>\n <li>Ability to communicate clearly and concisely in both written and verbal form.&nbsp;</li>\n <li>Ability to maintain accurate, legible and timely documentation and notes.</li>\n <li>Must be proficient in workplace English and spelling.</li>\n <li>Bilingual skills, including fluency in Spanish, preferred.</li>\n <li>Ability to read County policies and procedures; written instructions, general correspondence; MSDS sheets, safety manuals, maps, etc.</li>\n</ul>\n<p><strong>MATHEMATICAL SKILLS</strong></p>\n<ul>\n <li>Ability to perform basic mathematical and medication dosage calculations accurately.</li>\n</ul>\n<p><strong>REASONING ABILITY</strong></p>\n<ul>\n <li>Ability to understand and effectively carry out verbal and written instructions.</li>\n <li>Ability to understand, interpret and implement local policies and procedures;
