Transportation needs dependent upon supervisor.  </span></span></p>\n<p><span style=\"font-size: 12pt\"> </span></p>\n<p><span style=\"font-size: 12pt; font-weight: bold\"><span style=\"font-family: Arial, sans-serif\">ESSENTIAL DUTIES AND RESPONSIBILITIES:</span></span></p>\n<ul>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 12pt\">Keep the vehicle neat and tidy.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 12pt\">Keep gas tank full using company gas card.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 12pt\">Be willing to work with other staff for the coordination of patient care.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 12pt\">Courteous and professional at all times. </span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 12pt\">Other duties may be assigned. </span></li>\n</ul>\n<p><span style=\"font-family: Arial, sans-serif; font-size: 12pt\"> </span></p>\n<p><span style=\"font-size: 12pt; font-weight: bold\"><span style=\"font-family: Arial, sans-serif\">QUALIFICATIONS:</span></span></p>\n<ul>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 12pt\">Valid driver’s license.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 12pt\">Cleared by insurance.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 12pt\">Motor vehicle check.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 12pt\">This position requires employee to be 25yrs of age. </span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 12pt\">Random drug screen.</span></li>\n</ul>\n<p><br></p>\n<p><span style=\"font-size: 12pt; font-weight: bold\"><span style=\"font-family: Arial, sans-serif\">PHYSICAL DEMANDS:</span></span></p>\n<p><span style=\"font-family: Arial, sans-serif; font-size: 12pt\">Physical demands described are representative of those that must be routinely met by and employee to successfully perform the essential duties of this position.  Reasonable accommodations may be made to enable qualified individuals with disabilities to perform the essentials duties.</span></p>\n<ul>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 12pt\">Ability to lift minimum 25 pounds.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 12pt\">Ability to perform job responsibilities in a facility with multiple levels and without elevator or mechanical transportation.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 12pt\">Operates a computer and keyboard.</span></li>\n</ul>\n<p><br></p>\n<p><span style=\"font-size: 12pt; font-weight: bold\"><span style=\"font-family: Arial, sans-serif\">POSITION TYPE/EXPECTED HOURS OF WORK:</span></span></p>\n<p><span style=\"font-family: Arial, sans-serif; font-size: 12pt\">This is a full-time position.
