If you are passionate about assisting others, we want to hear from you.</span></p>\n<p><span style=\"font-size: 12pt; font-weight: bold\">Job Summary</span></p>\n<p><span style=\"font-size: 12pt\">To provide one-on-one in-home rehabilitative services to children, adolescents, and families with children with mental illness. Rehabilitative services enable clients to develop and enhance psychiatric stability, social competencies, personal and emotional adjustment, independent living, and community skills, and provide resources and referrals.</span></p>\n<p><span style=\"font-size: 12pt; font-weight: bold\">Duties and Responsibilities</span></p>\n<ul>\n<li><span style=\"font-size: 12pt\">Provide CTSS individual and family skills training in the child’s home, school or community that addresses the behaviors and areas of needed life skills to minimize the negative impact of the mental health diagnosis on daily living</span></li>\n<li><span style=\"font-size: 12pt\">Must have knowledge and understanding of mental illness</span></li>\n<li><span style=\"font-size: 12pt\">Must be able to travel within the Central Minnesota Region</span></li>\n<li><span style=\"font-size: 12pt\">Encourage client relationship and regular communication, coordinate services with other health care providers, interpreter, vocational workers, etc to ensure that client receives quality service to meet their needs</span></li>\n<li><span style=\"font-size: 12pt\">Guide clients in the development of skills and strategies for dealing with problems.</span></li>\n<li><span style=\"font-size: 12pt\">Support clients and family members to assist in adjusting to life and making changes</span></li>\n<li><span style=\"font-size: 12pt\">Maintain a caseload and be able to sustain attention to detail</span></li>\n<li><span style=\"font-size: 12pt\">Ability to work independently, be flexible and able to meet deadlines</span></li>\n<li><span style=\"font-size: 12pt\">Accurately document services and complete progress notes and submit within 72 hours</span></li>\n<li><span style=\"font-size: 12pt\">Develops and implements treatment plans based on clinical experience and knowledge Monitor and review treatment plans every 90 days.</span></li>\n<li><span style=\"font-size: 12pt\">Collaborate with other staff members to evaluate client’s progress in resolving identified problems and moving towards defined objectives</span></li>\n<li><span style=\"font-size: 12pt\">Evaluate clients’ progress Modify treatment activities and approaches as needed in order to comply with changes in clients’ status</span></li>\n<li><span style=\"font-size: 12pt\">Must attend clinical supervision as required by Medical Assistance guidelines</span></li>\n<li><span style=\"font-size: 12pt\">Ensure compliance with DHS regulations</span></li>\n<li><span style=\"font-size: 12pt\">Maintain confidentiality of records relating to clients’ treatment</span></li>\n<li><span style=\"font-size: 12pt\">Communicate effectively and respectfully with supervisor, office staff, clients, vendors, and clients</span></li>\n<li><span style=\"font-size: 12pt\">Mandated reporter</span></li>\n<li><span style=\"font-size: 12pt\">Proactively and efficiently communicating with clients, interpreters, and other parties if sessions are canceled or rescheduled</span></li>\n<li><span style=\"font-size: 12pt\">Must complete 15 hours per year of ongoing continuing education in areas of mental illness and mental health services</span></li>\n</ul>\n<p><span style=\"font-size: 12pt; font-weight: bold\">Scope of the Position </span></p>\n<p><span style=\"font-size: 12pt\">Under close clinical supervision of a Licensed Mental Health Professional, the mental health practitioner will provide in-home life skills training to individuals and families. Treatment goals will be developed to assist participants with psychosocial skills that are medically necessary to help the client reach an age appropriate developmental level.</span></p>\n<p><span style=\"font-size: 12pt; font-weight: bold\">Qualifications</span></p>\n<ul>\n<li><span style=\"font-size: 12pt\">Must meet DHS qualifications for Mental Health Practitioner in one of the following ways:</span></li>\n</ul>\n<p><span style=\"font-size: 12pt\">o Has completed at least 30 semester hours or 45 quarter hours in behavioral sciences</span></p>\n<p><span style=\"font-size: 12pt\">or related fields and has at least 2,000 hours of supervised experience in the delivery</span></p>\n<p><span style=\"font-size: 12pt\">of services to adults or children with:</span></p>\n<p><span style=\"font-size: 12pt\">A. Mental illness, substance use disorder, emotional disturbance, or</span></p>\n<p><span style=\"font-size: 12pt\">B. Traumatic brain injury or developmental disabilities and completes training on mental</span></p>\n<p><span style=\"font-size: 12pt\">illness, recovery from mental illness, mental health de-escalation techniques, co-occurring mental illness, and substance abuse, and psychotropic medications and side effects.</span></p>\n<p><span style=\"font-size: 12pt\">o Has at least 4,000 hours of supervised work experience in the delivery of services to adults or children with any of the following:</span></p>\n<p><span style=\"font-size: 12pt\">A. Mental illness, substance use disorder, emotional disturbance, or</span></p>\n<p><span style=\"font-size: 12pt\">B. Traumatic brain injury or developmental disabilities and completes training on mental</span></p>\n<p><span style=\"font-size: 12pt\">illness, recovery from mental illness, mental health de-escalation techniques, co-occurring mental illness, substance abuse, and psychotropic medications and side effects.</span></p>\n<p><span style=\"font-size: 12pt\">o Has at least 2,000 hours of supervised experience in the delivery of services to adults</span></p>\n<p><span style=\"font-size: 12pt\">or children with any of the following:</span></p>\n<ul>\n<li><span style=\"font-size: 12pt\">Mental illness, substance use disorder, emotional disturbance, or</span></li>\n<li><span style=\"font-size: 12pt\">Traumatic brain injury or developmental disabilities and completes training on mental illness, recovery from mental illness, mental health de-escalation techniques, co-occurring mental illness, substance abuse, and psychotropic medications and side effects; and receives clinical supervision as required at least once a week from a mental health professional until the requirement of 4,000 hours of supervised experience is met.</span></li>\n</ul>\n<p><span style=\"font-size: 12pt\">o Has a graduate student internship or a bachelor’s or master’s degree and is a graduate student in behavioral sciences or related fields and is formally assigned by an accredited college or university to an agency or facility for clinical training</span></p>\n<p><span style=\"font-size: 12pt\">o Has a bachelor’s or master’s degree with any of the following:</span></p>\n<p><span style=\"font-size: 12pt\">A. Holds a master’s or other graduate degree in behavioral sciences or related fields</span></p>\n<p><span style=\"font-size: 12pt\">B.
