Our mission is to provide compassionate, patient-centered care to individuals with serious illness while supporting families and caregivers. We are seeking an experienced Nurse Practitioner to help develop and drive the plan of care for palliative care for our existing hospice program.</p><p style=\"margin-left:0in;\"><strong>Position Summary:</strong><br>&nbsp;The Palliative Care Nurse Practitioner (NP) will provide high-quality palliative care services across home and facility settings while supporting hospice operations. This role includes conducting hospice face-to-face (F2F) encounters in compliance with CMS and New Jersey regulations, ensuring accurate documentation, and supporting appropriate billing and coding practices.
