La Clinica de Familia (LCDF) is a FQHC with several locations in Southern New Mexico. For over 40 years, La Clinica has provided services to the residents of Southern New Mexico. Exempt Up to $121,367 Job Summary: Under the supervision of the CEO, the Director of Population Health is a key leader in La Clinica de Familia’s (LCDF) movement towards an outcomes-focused, team-based care model whose goal is to provide improved health equity, quality, and cost. The Director of Population Health drives strategic oversight of population health programs, including Chronic Care Management, Certified Community Behavioral Health Clinic (CCBHC) Care Coordination, and Community Health Worker (CHW) initiatives, to deliver high-quality, patient-centered care, advance value-based performance, and ensure compliance with regulatory requirements.
