As a <strong>Home Health RN/RN Case Manager </strong>with UnityPoint at Home, you’ll provide one-on-one care to patients with total autonomy over your role. With a flexible schedule and independence, our home care nurses are in charge of everything from patient admission to the care plan to discharge and make it possible for patients to receive care where they’re most comfortable – in their homes. </p>\n<p> </p>\n<p>The Home Health RN/RN Case Manager uses clinical nursing knowledge, physical assessment, teaching and procedural skills to deliver high quality patient care in the patient’s place of residence. The Home Care RN develops and oversees the home health plan of care under the direction of the physician and ensure appropriate interdisciplinary involvement to ensure the best outcome for each patient.
