To alleviate poverty, we deliver a variety of services including childcare, education, workforce development, nutrition, health, and community and housing support.</p><p><strong>Position Overview:</strong></p><p>The Housing Case Manager is responsible for determining service plan goals, targets, and action steps for clients.&nbsp;</p><p><strong>Essential Duties &amp; Responsibilities:</strong></p><ul><li>Meeting with clients in-person or virtually to conduct an intake assessment that includes collection of information including demographic, financial, and social services information</li><li>Upon completion of the intake assessment, the HCM will document all assessments accurately and will ensure the payer approves the services</li><li>Following the initial assessment, the HCM will also be responsible for the case management of pre-tenancy services, tenancy-sustaining services, and for the identification of home modification needs</li></ul><p><strong>Required Skills and Qualifications:</strong></p><ul><li>Associate&#39;s degree in human services or related field</li><li>1 year of experience providing case management services focused on pre-tenancy and tenancy-sustaining services</li><li>Must have a reliable telephone&nbsp;</li><li>Must be empathetic to the needs of a diverse socio-economic population</li><li>Strong interpersonal skills&nbsp;</li><li>Knowledge of community resources, state and federal benefits and services</li><li>Strong project management skills with the ability to administer multiple cases simultaneously and prioritize workload.</li><li>Basic math and budgeting skills&nbsp;</li><li>Ability to work effectively as a team member with other members of program and agency leaders&nbsp;</li><li>Demonstrated prior success in case management functions or eligibility determinations for community-based social service organizations&nbsp;</li><li>Problem-solving abilities&nbsp;</li><li>Ability to communicate and collaborate with health providers and community partners&nbsp;</li><li>Strong internet, database and computer skills</li><li>Good documentation skills&nbsp;</li><li>Ability to read and write Spanish preferred</li></ul><p><strong>Physical Requirements:</strong>&nbsp; &nbsp;</p><ul><li>Prolonged periods of sitting at a desk and working on a computer</li><li>Must be able to lift up to 15 pounds at times</li></ul><p><strong>Other duties:</strong></p><p>Please note this job description is not designed to cover or contain a comprehensive listing of activities, duties, or responsibilities that are required of the employee for this job.&nbsp;</p><p><strong>Compensation:</strong></p><p>A salary within the range of $24.51 to $33.17 per hour will be provided to the successful candidate having met the minimum requirements for the position consistent with the MOC compensation philosophy.
