CST provides mental health rehabilitation services designed to decrease hospitalization and crisis episodes and to increase community functioning so individuals can achieve their rehabilitative, resiliency, and recovery goals.</p><p data-sourcepos=\"10:1-10:303;526-828\">Working under the clinical direction of a full-time QMHP team lead, the Community Support Specialist delivers individualized, community-based services that facilitate illness self-management, skill building, and connection to community resources &mdash;
