Program: Preventive (Brooklyn/ Bronx /Harlem)

Provides and coordinates excellent quality care and healing to Graham Windham’s children and ensures that every one of the children in the program is living in a nurturing, safe, and permanent family as quickly as possible. Duties and Responsibilities include but not limited to :
· Provide onboarding support to new case planning staff.
· Shadow new Case Planners to home visits and mandated conferences during their first three months of employment.
· Provide technical assistance as it relates to navigating Connections, SharePoint and PROMIS.
· Provide additional assistance and guidance to Case planners as it relates to the completion of Sex Trafficking Assessments, Domestic Violence, Trauma, and CAGE Screenings, etc.
· Support new Case planners in the implementation and continuous utilization of the Solution Based Casework (SBC) model in the absence of a supervisor.
· Manage a caseload of up to 8 families.
· Attend and facilitate internal and external meetings/trainings at the direction of the Program Director.
· Create with families’ service plans that are inclusive of stakeholders’ input; build on clients’ strengths; that are realistic, culturally competent, provide meaningful assistance, and are in accordance with City, State and Federal Regulations.
· Ensures that children are safe and well cared for in their homes through development and maintenance of a positive working relationship with families and their support network.
· Respond immediately to early signs of safety and elevated risk concerns.
· Gauges bonding between child and parents through careful observation of parent/child interaction during home visits and from feedback from other service providers.
· Attends & plays an active role in Safety, Elevated Risk, Service Termination and Family Team Conferences.
· Assist with home and collateral contacts when there are case planning vacancies.
· Responds to client emergencies within or outside work hours.
· Other duties as assigned by Supervisor.
Qualifications
Requires BSW or BA degree in Social Work, Sociology, Psychology, or related field, Bilingual in S panish preferred; (2) years minimum preventive experience required. Must be able to travel within NYC Metropolitan area; Must have excellent verbal and written communication skills, time management and organizational skills; Computer literate, MS Office; NYS Connections; Ability to work with diverse populations.
