You'll coordinate with third-party payers, government programs, and community services to arrange appropriate care, housing, transportation, and other essential services. Your role will involve coordinating placement and support services, including long-term care, rehabilitation, behavioral/mental health, home care, transportation, and DME, as well as assisting with CPS, APS, adoptions, and end-of-life planning, including advance directives. This position offers an opportunity to make a direct impact on patient care by helping navigate complex discharge processes and fostering continuity of care.</p><p style=\"line-height: normal; margin-bottom: 0in; margin-top: 0in;\"><strong>How you’ll contribute</strong></p><p style=\"line-height: normal; margin-bottom: 8pt;\">A Case Manager Social Worker who excels in this role:</p><ul style=\"list-style-type: disc;\"><li><p style=\"line-height: normal; margin-bottom: 0in;\">Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives.</p></li><li><p style=\"line-height: normal; margin-bottom: 0in;\">Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.</p></li><li><p style=\"line-height: normal; margin-bottom: 0in;\">Participates in case reviews to evaluate case management and progress. Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan.</p></li><li><p style=\"line-height: normal; margin-bottom: 0in;\">Assists with discharge planning and processes.
