This program will provide permanent supportive housing and case management services for adults living with HIV/AIDS and chronic medical conditions as well as Young Adults (18-24) aging out of foster care.&nbsp;<em id=\"isPasted\"><strong>This position offers a salary range of $45,000-$50,000, with a generous benefit package including health insurance, dental, vision, dependent care, paid time off, 401(k), tuition assistance program and more.</strong></em></p><p><strong>Qualifications:</strong></p><ul><li>Master&rsquo;s Degree in Social Work, Human Services or related field</li><li>New York State License Master Social Work (LMSW) or New York State License Mental Health Counselor (LMHC)</li><li>Credentialed Alcohol and Substance Abuse Counseling (CASAC-T or CASAC) a plus</li><li>Knowledge of HRA Congregate Housing Program guidelines</li><li>One (1) to Three (3) years&rsquo; Mental Health work experience</li><li>Mental Health service experience with individual and families with living with HIV/AIDS, substance abuse, serious mental illness, and homelessness</li><li>Bilingual English/Spanish a plus</li><li>5 Day/35 work week Non-Remote Position (In-Office/Some Field Work)</li><li>Excellent oral and written communication skills</li><li>Proficient in Google Workspace program/Microsoft Office Suite</li><li>Familiar with AWARDS, Child and Children Integrated Reporting System (CAIRS), &nbsp;Coordinated Assessment and Placement System (CAPS)</li></ul><p><strong>Responsibilities:</strong></p><ul><li>Conduct mental health assessments</li><li>Provide supportive counseling to clients</li><li>Provide mental health services and referrals&nbsp;</li><li>Coordinate and collaborate with mental health providers&nbsp;</li><li>Conduct home visits and maintain face to face contact with clients</li><li>Assist in the development and implementation of comprehensive service plans</li><li>Develop and implement weekly groups for clients</li><li>Completion of required assessment reporting tools and progress notes</li><li>Participate in case conferences, training, workshops and seminars</li><li>Assist in the planning and organizing program recreational activities</li><li>Participates in weekly supervision</li></ul><p><br></p><div><link href=\"https://static.workforcenow.adp.com/mas/mdf-components/22.3.26/styles/froala_style.min.css\" rel=\"stylesheet\" type=\"text/css\"><div class=\"fr-view\"><p dir=\"ltr\" id=\"isPasted\" style=\"line-height:1.295;margin-top:0pt;margin-bottom:8pt;\"><br></p></div></div></div>\n    </div>\n  </div></div></div></div></div>\n",
