With over 200 employees we strive to provide high-quality and innovative health care services to the Finger Lakes Region.</span></p>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Seeking a qualified NYS  Licensed Mental Health Counselor (LMHC), License Clinical Social Worker (LCSW), or Licensed Master of Social Work (LMSW) Hybrid remote position with several locations to choose from.</span></p>\n<p> </p>\n<p><span style=\"text-decoration: underline\"><span style=\"font-weight: bold\"><em>Benefits Offered:</em></span></span></p>\n<ul>\n<li>Monday through Friday schedule, closed all major holidays</li>\n<li>Medical insurance with a 90% employer contribution</li>\n<li>Dental, Vision, and Life insurance</li>\n<li>Safe Harbor 3% 401k contribution</li>\n<li>Robust PTO offerings </li>\n<li>Education reimbursement </li>\n</ul>\n<p><br></p>\n<p><span style=\"text-decoration: underline\"><span style=\"font-weight: bold\"><em>Job Summary:</em></span></span></p>\n<p>Responsible for providing psychotherapy, clinical assessment and evaluation to identify and treat disabilities and problems of behavioral, developmental, emotional, or social functioning in individuals served at Finger Lakes Community Health in accordance with the New York State Department of Health Article 28 regulations for Diagnostic and Treatment Center. </p>\n<p><br></p>\n<p><span style=\"text-decoration: underline\"><span style=\"font-weight: bold\"><em>Main Responsibilities:</em></span></span></p>\n<p><br><em><span style=\"font-weight: bold\">Evaluate and complete assessment</span></em></p>\n<ul>\n<li><br>Provide verbal talk therapy or behavioral methods to treat disorders of behavior, character, emotional, or personality.</li>\n<li>Use assessment instruments in addition to mental health counseling to identify and evaluate disorders and dysfunctions of mental illness.</li>\n</ul>\n<p><br><em><span style=\"font-weight: bold\">Utilize various therapeutic inventions</span></em></p>\n<ul>\n<li><br>Provide mental health counseling using evidence based approaches (this will be up to provider and patient needs). This may include but not limited to:\n<ul>\n<li>Cognitive Behavioral Therapy</li>\n<li>Dialectical Behavioral Therapy</li>\n<li>Solution Focused Therapy</li>\n<li>Motivational Interviewing</li>\n</ul>\n</li>\n</ul>\n<p><span style=\"font-weight: bold\"><em>Develop and implement treatment plan</em></span></p>\n<ul>\n<li>Work collaboratively with individual on individualized treatment plan that includes the following:\n<ul>\n<li>Diagnosis/assessment</li>\n<li>Goal</li>\n<li>Objective</li>\n<li>Barriers to treatment</li>\n<li>Strength of individual</li>\n<li>Modality and Intervention used</li>\n<li>Target date</li>\n</ul>\n</li>\n</ul>\n<p><br><em><span style=\"font-weight: bold\">Coordinate care with an integrated focus</span></em></p>\n<ul>\n<li><br>Team based coordination with primary care</li>\n<li>Participate in behavioral health case conference</li>\n<li>Be a core member with a multidisciplinary care team</li>\n<li>Participate in a planned supervision for license and consultation</li>\n</ul>\n<p><em><span style=\"font-family: Lato, sans-serif; font-size: 15px; font-weight: bold\">Documentation</span></em></p>\n<ul>\n<li><span style=\"font-family: Lato, sans-serif; font-size: 15px\">Progress notes and treatment plans are documented in an integrated electronic medical record. Notes will include the following but not limited to:</span>\n<ul>\n<li><span style=\"font-family: Lato, sans-serif; font-size: 15px\">Presenting problem and status of symptoms</span></li>\n<li><span style=\"font-family: Lato, sans-serif; font-size: 15px\">Treatment goals</span></li>\n<li>Progress towards goals</li>\n<li>Functional status</li>\n</ul>\n</li>\n</ul>\n<p>*Depending on license, documentation may require supervision and co-signature of notes by a Licensed Clinical Social Worker (LCSW)</p>\n<p><br></p>\n<p><span style=\"text-decoration: underline\"><span style=\"font-weight: bold\"><em>Education and Qualifications:</em></span></span></p>\n<ul>\n<li><span>A current unrestricted NYS Licensed required</span></li>\n<li><span style=\"font-weight: bold\">Required: The ability to communicate fluently in both Spanish and English</span></li>\n<li><span>CPR certification required (can be provided) </span></li>\n<li><span>Strong interpersonal skills and the ability to build relationships with personnel, patients, and community partners</span></li>\n<li><span>Proven ability to handle confidential information with discretion</span><br></li>\n<li><span>The ability to work flexible hours -- some evenings are required</span></li>\n</ul>\n<p><br></p>\n<p><span style=\"text-decoration: underline\"><span style=\"font-weight: bold\"><em>Physical Requirements: </em></span></span></p>\n<ul>\n<li><span>Must be able to hear and communicate with clients and staff via phone, web media, and in person.</span></li>\n<li><span>Must be able to lift up to 50 lbs</span></li>\n<li><span>Must be able to sit for extended periods of time</span></li>\n<li><span>OSHA Category I</span></li>\n</ul>\n<p><br></p>",
