Under the direct supervision of the Division Director, the Child Advocate LPN will perform healthcare coordination services to children in the Special Medical Program. The Special Medical Team uses a trauma-informed approach to ensure that all children placed in HSVS&rsquo;s special medical homes are receiving all needed medical, dental, behavioral health, and specialty health services through HSVS&rsquo;s 29-I primary care clinic and Medicaid Managed Care Plan (MMCP) in-network providers, and that these services are being documented appropriately in the NextGen Electronic Health Record (EHR). The LPN will be an integral part of this growing Special Medical Team and will also work closely with the assigned case planning team.</span></span></p>\r\n\r\n<p><span style=\"display:block;font-size:14px;\"><span style=\"font-family:Arial,Helvetica,sans-serif;\"><strong>Primary Responsibilities and Essential Functions of Position:</strong></span></span></p>\r\n\r\n<ul>\r\n\t<li><span style=\"display:block;font-size:14px;\"><span style=\"font-family:Arial,Helvetica,sans-serif;\">Coordinate all aspects of healthcare services for designated caseload of special medical clients.
