It is the goal of The Black Mental Health Corporation to<br>rebuild these communities, one family, at a time through excellent mental health and social<br>assistance services.</p>\n<p><br>Our History:<span> </span>TBMHC was created by Desmond Yanamayu with people of color in mind, after<br>realizing this diverse population experienced challenges and struggles that were unique from<br>many other racial/ethnic groups. Because of this, he wanted to create a space where black people<br>could feel comfortable getting counseling with someone who understood issues specifically<br>related to them. Even still, The Black Mental Health Corporations’ services are open to people<br>of all ethnicities and racial backgrounds.</p>\n<p><br>Essential Duties and Responsibilities:<br>• Facilitate initial procedures for admission, including completion of required intake forms<br>and releases.<br>• Complete assessments on all clients/families applying eligibility and diagnostic criteria to<br>determine client appropriateness for treatment in various modalities.<br>• Facilitate individual, group, and family counseling, facilitate client and family groups<br>and programs, and facilitate other groups as assigned.<br>• Provide crisis intervention and risk assessments as needed.<br>• Develop specific treatment goals in collaboration with the client, client’s guardian, and<br>client’s treatment providers.<br>• Identify and evaluate client’s strengths, limitations, problems, and needs and includes<br>these in the client ISP (individual service plan).<br>• Collaborate with the treatment team to develop, implement and modify the ISP as required.<br>• Participate in treatment team to provide updates on client progress in group, individual,<br>and family counseling.<br>• Complete treatment plans and all required documentation; submit as required.<br>• Meet productivity requirements.<br>• Provide a variety of counseling services to address the specific needs of the population<br>served. Skilled in working with clients with multiple issues: trauma, addiction, mental<br>health, grief, loss, etc.<br>• Utilize evidence-based treatment to ensure effective service delivery based on the client’s<br>clinical issues/needs.<br>• Provide information and guidance on utilization of community resources, initiates and<br>follows up on referrals as needed.<br>• Chart results of assessment and treatment planning; writes reports, progress notes,<br>discharge summaries, and other client-related information in support of the client’s progress<br>in treatment in a timely manner.<br>• Participate actively in the supervision process to ensure comprehensive, quality care for<br>clients.<br>• Participate in professional development by attending workshops, seminars, and classes in<br>job-related areas.<br>• Comply with all federal, state, and funding regulatory requirements.<br>• Performing other related or requested activities as assigned.</p>\n<p><br></p>\n<p>Education and License Requirements</p>\n<ul>\n<li>Masters Level education in Social Work, Psychology, or field related to Mental Health.</li>\n<li>Licensed by the State of Ohio (Preferred LISW, LPCC, IMFT)</li>\n</ul>\n<p>#zr</p>",
