    "description": "<p>GENERAL PURPOSE:&nbsp;</p>\r\n\r\n<p>The social worker is responsible for the implementation of standards of care for medical social services and understanding the community development principles and practices, with a focus on building and sustaining community partnerships.&nbsp;&nbsp;</p>\r\n\r\n<p>QUALIFICATIONS:&nbsp;</p>\r\n\r\n<ul>\r\n\t<li>\r\n\t<p>Bachelor&rsquo;s or master&rsquo;s degree in social work from a state accredited program.&nbsp;&nbsp; If has a bachelor&rsquo;s degree and hired after 12/2/08, mut be supervised by a Master of Social work.&nbsp;</p>\r\n\t</li>\r\n</ul>\r\n\r\n<ul>\r\n\t<li>\r\n\t<p>Minimum of one (1) years&rsquo; experience in health care.&nbsp;</p>\r\n\t</li>\r\n</ul>\r\n\r\n<ul>\r\n\t<li>\r\n\t<p>Experience in Hospice/home health care preferred.&nbsp;</p>\r\n\t</li>\r\n</ul>\r\n\r\n<ul>\r\n\t<li>\r\n\t<p>Demonstrates good verbal and written communication, and organization skills.&nbsp;</p>\r\n\t</li>\r\n</ul>\r\n\r\n<ul>\r\n\t<li>\r\n\t<p>Possesses and maintains current CPR certification.&nbsp;</p>\r\n\t</li>\r\n</ul>\r\n\r\n<ul>\r\n\t<li>\r\n\t<p>Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.&nbsp;</p>\r\n\t</li>\r\n</ul>\r\n\r\n<ul>\r\n\t<li>\r\n\t<p>Must be capable of performing the job functions of this position with or without accommodations.&nbsp;</p>\r\n\t</li>\r\n</ul>\r\n\r\n<p>&nbsp;</p>\r\n\r\n<p>ESSENTIAL JOB FUNCTIONS:&nbsp;</p>\r\n\r\n<ul>\r\n\t<li>\r\n\t<p>Assesses the psychosocial status of patients related to the patient&rsquo;s illness and environment and communicates findings to the registered nurse.&nbsp;</p>\r\n\t</li>\r\n\t<li>\r\n\t<p>Carries out social evaluations and plans intervention based on evaluation findings.&nbsp;</p>\r\n\t</li>\r\n\t<li>\r\n\t<p>Maintains clinical records on all patients referred to social work.&nbsp;</p>\r\n\t</li>\r\n\t<li>\r\n\t<p>Provides information and referral services for Company patients and families/caregivers regarding practical and environmental needs.&nbsp;</p>\r\n\t</li>\r\n\t<li>\r\n\t<p>Provides information to patients or families/caregivers and community agencies.&nbsp;</p>\r\n\t</li>\r\n\t<li>\r\n\t<p>Serves as liaison between patients or families/caregivers and community agencies.&nbsp;</p>\r\n\t</li>\r\n\t<li>\r\n\t<p>Maintains collaborative relationships with Company personnel to support patient care.&nbsp;</p>\r\n\t</li>\r\n\t<li>\r\n\t<p>Work in partnership with sales team to develop strategies to increase community involvement and engagement.&nbsp;</p>\r\n\t</li>\r\n\t<li>\r\n\t<p>Collaborate with sales team members to promote the organization&rsquo;s services and community activities.&nbsp; Along with representing the organization at community events.&nbsp;</p>\r\n\t</li>\r\n\t<li>\r\n\t<p>Maintains and develops contracts with public and private agencies as resources for patients and Company personnel.&nbsp;</p>\r\n\t</li>\r\n\t<li>\r\n\t<p>Participates in the development of the total plan of care and case conferences as required.&nbsp;</p>\r\n\t</li>\r\n\t<li>\r\n\t<p>Assists physicians and other team members in understanding significant social and emotional factors related to health problems.&nbsp;</p>\r\n\t</li>\r\n\t<li>\r\n\t<p>Participates in discharge planning.&nbsp;</p>\r\n\t</li>\r\n\t<li>\r\n\t<p>Adheres to all of the Company&rsquo;s policies and procedures.&nbsp;</p>\r\n\t</li>\r\n\t<li>\r\n\t<p>Perform other duties as assigned.&nbsp;</p>\r\n\t</li>\r\n</ul>\r\n\r\n<p>The above statements are only meant to be a representative summary of the major duties and responsibilities performed by incumbents of this job.
