Position Overview Services for the UnderServed (SUS) is a New York-based nonprofit organization that provides housing and support services to 25,000 individuals and their families living in New York City and Long Island. For more than 35 years, SUS has been transforming this belief into reality by working as an advocate for people with life circumstances marked by intellectual/developmental disability, behavioral health challenges, poverty, & histories of incarceration and/or trauma. We drive scalable solutions to transform the lives of people with disabilities, people in poverty and people facing homelessness: solutions that contribute to righting societal imbalances. Our commitment to continuous quality improvement impacts the lives of our clients and communities while contributing to the development of our employees. Position Summary: The QIDP will coordinate all aspects of care for residents in order to ensure active treatment is provided on an ongoing basis in the least restrictive environment, in accordance with OPWDD regulations Responsibilities: Coordinate and conduct Annual and Semi-Annual reviews of consumer status/progress including an assessment of risks to consumer’s rights and protection Ensure completion and receipt of individual clinical team members’ reports/meetings evaluations Prepare annual CFA’s and summaries which includes a capabilities needs assessment, priority of needs and a summary of clinical reports, activities schedules and consumer profiles Ensure the least restrictive interventions or practices are utilized Ensure individuals identified as having practices, interventions and or adaptive equipment, which may pose a risk to consumer’s rights and or protection, are presented to the Human Rights Committee for their review and or approval Observe Program Specialist conduct goals active treatment with and provide hands-on training as needed or prescribed consumers by facilities Plan Of Corrective Action (POCA) Provide staff with in-service training ongoing Ensure integration of services between Day Program and the residence and, integration of all services into routine daily activities Complete CSET schedule annually and update as needed. Also ensure, CSET training is provided and reported on by clinicians as needed Complete Quality Improvement reports as needed and ensure resolution to any outstanding issues Maintain communication with family/correspondent in regards to residential programming in conjunction with Social Worker Benefits Overview: We offer attractive compensation with comprehensive benefits including: Medical/Dental/Prescription/Vision/Life Insurance;403(b); Credit Union; FSAs; Short-and-Long-Term Disability; Transportation Plan; Generous Paid Vacations and Holidays Qualifications BA Degree in Psychology, Human Services or related field. 2-5 years experience within a Day Hab or Residential program.
